健脾理气抑瘤方联合CIK治疗对晚期肝细胞癌患者免疫功能及生活质量的影响

摘　　要：目的研究健脾理气抑瘤方联合细胞因子诱导的杀伤细胞(CIK)治疗对晚期肝细胞癌(HCC)患者免疫功能和生活质量的影响。方法将60例晚期HCC患者根据是否愿意服用健脾理气抑瘤方分为观察组和对照组,观察组接受健脾理气抑瘤方联合CIK治疗治疗,对照组接受辨证中药联合CIK治疗。通过T细胞亚群及EORTC QLQ-C30评分量表,观察两组治疗前后免疫功能及生活质量的变化。结果观察组治疗后外周血T细胞亚群中的CD3^+、CD4^+、CD8^+、NK^+细胞比例较治疗前升高,对照组的CD3^+、CD4^+、NK^+细胞比例较治疗前升高,两组的CD3+差值比较差异有统计学意义(P<0.05)。观察组治疗后躯体功能、情感功能、总健康评分较前增加,疲倦、恶心呕吐、气短、食欲、便秘、腹泻条目评分较前下降;对照组治疗后躯体功能评分较前增加,疲倦条目评分较前下降;两组的总健康和疲倦差值比较差异有统计学意义(P<0.05)。治疗期间两组患者未见严重不良反应。结论中药联合CIK治疗能改善晚期HCC患者的免疫功能和生活质量;健脾理气抑瘤方联合CIK治疗效果更优。Objective To observe the efficacy of Jianpi Liqi Yiliu Formula（ JLYF） combined with cytokine- induced killer（ CIK） cells on the immune function and quality of life in patients with advanced hepatocellular carcinoma（ HCC）. Methods Sixty patients were randomly assigned to receive either JLYF and CIK treatment（ the treatment group） or dialectical decoction and CIK treatment（ the control group）. The immune function and quality of life（ QOL）were evaluated by T lymphocyte subset assay and EORTC QLQ- C30 score. Results The T lymphocyte subset assay showed that the percentages of CD3~＋,CD4~＋,CD8~＋and NK~＋were increased after treatment in the treatment group,as well as in the control group. There was a statistically significant difference in the CD3＋difference before and after treatment between two groups（ P 0. 05）. The scores of physical function,emotional function and general health were increased and the scores of fatigue,nausea,short of breath,appetite,constipation,diarrhea were decreased after treatment in the treatment group（ P 0. 05）. The score of physical function was increased and score of fatigue was decreased after treatment in the control group（ P 0. 05）. With regards to scores of general health and fatigue,there was a statistically significant difference in the difference before and after treatment between two groups（ P 0. 05）. No severe adverse events were observed. Conclusion Both treatment strategies can enhance the immune function and QOL,while JLYF combined with CIK treatment is more effective than dialectical decoction combined with CIK treatment.
