胃镜下注射治疗消化性溃疡并急性上消化道出血的疗效分析

摘　　要：目的探讨胃镜下注射治疗消化性溃疡并急性上消化道出血的疗效。方法收集我院2013年4月至2015年4月入院的100例消化性溃疡并急性上消化道出血患者随机分为两组。对照组患者给予胃镜下病灶处药物喷洒治疗，实验组患者给予胃镜下病灶处药物注射治疗。比较两组患者治疗前后血红蛋白含量、总体治疗效果评价、相关临床指标与不良反应。结果实验组患者治疗后Hb水平（108．56±19．77）g／L显著高于对照组（96．17±15．89）g／L，差异有统计学意义（P〈0．01）。实验组患者治疗总有效率（96％）明显高于对照组（84％），差异有统计学意义（P〈0．05）。实验组患者止血时间（4．58±1．22）d、住院时间（8．76±3．25）d与大便频繁发生率0％、血压紊乱发生率0％显著低于对照组（8．46±3．09）d、（13．34±5．11）d、22％、12％，差异有统计学意义（P〈O．05）。结论胃镜下注射治疗消化性溃疡并急性上消化道出血的疗效显著，具有借鉴性。Objective To investigate the efficacy of gastroscopy injection in the treatment of peptic ulcer complicated with acute upper gastrointestinal bleeding. Methods 100 cases of peptic ulcer complicated with acute upper gastrointestinal bleeding admitted to our hospital from April 2013 to April 2015 were collected and randomly divided into two groups. Control group was treated with medical sprinkling on the lesions under gastroscope; while experimental group was treated with medical injection in the lesions under gastroscope. Hemoglobin （Hb） content, relevant clinical indicators before and after treatment, the evaluation of overall treatment effect, and adverse reactions were compared between two groups. Results Hb level of experimental group after treatment was significantly higher than that of control group [（108.56±19.77）g/L vs.（96.17±15.89） g/L], with statistically significant difference （P〈0.01）. The overall treatment effective rate of experimental group was significantly higher than that of control group （96% vs.84%）, with statistically significant difference （P〈0.05）. The hemostatic time, hospital stays, the incidence of frequent stool, the incidence of chaotic blood pressure of experimental group were significantly lower than those of control group [（4.58±1.22）d vs.（8.46±3.09） d, （8.76±3.25）d vs.（13.34±5.11）d, 0% vs.22%, 0% vs.12%], with statistically significant differences （P〈0.05）. Conclusion The efficacy of gastroscopy injection in the treatment of peptic ulcer complicated with acute upper gastrointestinal bleeding is significant, of reference in clinical practice.
