摘 要:目的 探讨不同入路显微外科手术治疗基底节区高血压脑出血的临床疗效。方法 随机选取我院2013年4月1日~2015年10月1日收治入院的120例基底节区高血压脑出血患者作为研究对象。按随机数字表法将其平均分为两组。观察组患者给予经外侧裂-岛叶入路显微外科手术治疗;对照组患者给予经颞叶皮层入路显微外科手术治疗,观察比较两组患者血肿清除率、不良反应发生率及治疗前后格拉斯哥昏迷评分(GCS)。结果 观察组患者血肿清除率≥95%、80%≤血肿清除率〈95%、血肿清除率〈80%的百分比分别为75.00%、23.33%、1.67%,对照组为48.33%、28.33%、23.33%;观察组不良反应总发生率为10.00%,对照组为23.33%;观察组治疗后GCS评分为(13.57±1.05)分,对照组为(10.87±2.34)分。观察组患者各项相关指标效果均优于对照组患者,超早期治疗效果更优,差异有统计学意义(P〈0.05)。结论 经外侧裂-岛叶入路显微外科手术治疗基底节区高血压脑出血患者,临床疗效显著,且早期治疗效果更佳。应在早期积极给予经外侧裂-岛叶入路显微外科手术治疗,提高临床疗效。Objective To investigate the clinical effect of microsurgical treatment of hypertensive cerebral hemorrhage in basal ganglia area with different approaches. Methods 120 patients with hypertensive cerebral hemorrhage in basal ganglia region admitted into our hospital from 1^st April 2013 to 1^st October 2015 were randomly selected as research objects.The data were ranged.They were divided into two groups based on a random number table.In the observation group,microscopic surgery via lateral fissure-insular basal approach was used,while in the control group,microscopic surgery via temporal cortex approach was adopted.The hematoma cleai:ance rate,incidence of adverse reactions as well as Glasgow coma score before and after treatment (GCS) were observed and compared between the two groups. Results In the observation group,percentages of hematoma clearance rate ≥195%,80%≤〈hematoma clearance rate〈95% and hematoma clearance rate〈80% were 75.00%,23.33%,and 1.67%.In the control group,they were 48.33%,28.33%,and 23.33% in turn.The overall incidence of adverse reactions was 10.00% in the observation group,while in the control group,the incidence was 23.33%.In the observation group,score of GCS after therapy was (13.57±1.05) scores,while in the control group,the score was (10.87±2.34) scores.Every index in the observation group was superior to that in the control group,if treated in an earlier time,the effect would be much better,which displayed a statistical difference (P〈 0.05). Conclusion For patients with hypertensive cerebral hemorrhage in basal ganglia region,application of microscop- ic surgery via lateral fissure-insular basal approach obtains a remarkable effect in clinic,if treated in an earlier time,the effect will be much better.In the clinical treatment,positive microscopic surgery via lateral fissure-insular basal ap- proach at an early stage should be applied aiming at improve clinical efficacy.
