12例低颅压综合征患者的诊治总结

摘　　要：目的 探讨低颅压综合征患者的临床表现、影像学特征及诊治方法。方法 回顾性分析我院2013年6月~2015年6月收治的12例低颅压综合征患者的临床表现、影像学检查及诊治经过。结果 12例患者中10例患者首发症状是头痛和呕吐,2例首发症状是头晕伴听力下降,1例患者双侧外展神经受损,1例患者面神经受损。影像学检查10例患者硬脑膜均有强化,2例提示双侧额颞叶硬膜下积液,1例患者头颅MRI提示双侧额顶叶慢性硬膜下血肿,1例提示左侧额颞叶枕部及左侧小脑硬膜下出血,1例提示蛛网膜下腔增宽,1例提示小脑扁桃体下疝形成合并静脉窦血栓形成。腰穿压力检测5例压力正常,7例压力〈70 mm H_2O,所有患者均经保守治疗后好转出院。结论 熟悉并掌握低颅压综合征的临床表现及辅助检查特点,可帮助临床医生早期诊断并采取积极治疗。Objective To study the clinical feature,imaging characteristics and treatment and diagnosis of patients with low cranial pressure syndrome. Methods Clinical feature,imaging characteristics and treatment and diagnosis of 12 pa- tients with low cranial pressure syndrome were retrospectively analyzed. Results Among 12 patients,10 patients first symptom was headache and vomiting;2 patients first symptom was dizziness with hearing loss;1 patients with bilateral abducens nerve damaged, l with facial nerve damaged.Imaging examination showed 10 patients with dura mater strengthen,2 patients with bilateral frontal temporal lobe subdural effusion,1 patient cranial MRI showed with prompt bilateral frontal and parietal chronic subdural hematoma, 1 patient showed the left frontal temporal lobe left cere- bellum and occipital subdural hemorrhage, 1 patient showed prompted widened subarachnoid space and 1 patient showed under cerebellar tonsillar herniation combined venous sinus thrombosis, Lumbar puncture pressure detection showed 5 patients with normal pressure,7 cases with pressure〈70 mmH20 column.All patients discharged by using con- servative treatment. Conclusion Familiar with and master the clinical manifestations of lower cranial pressure syndrome and auxiliary examination can help doctors early diagnosis and adopt positive treatment.
