摘 要:目的:探讨联合应用磁共振弥散加权成像技术(diffusion weighted imaging,DWI)和磁共振波谱技术(magnetic resonance spectroscopy,MRS),对乳腺肿块鉴别诊断的价值。方法:回顾性分析经病理证实的66例患者乳腺结节或肿块,按事先确定的诊断标准,对病变进行分类统计,观察单一技术和联合两种技术对乳腺肿块鉴别诊断的敏感性和特异性,并分析其统计学差异。结果:66例患者中,共发现81个乳腺结节或肿块,其中恶性43个,良性38个。DWI诊断敏感性和特异性分别为72.1%和65.8%;MRS诊断敏感性和特异性分别为65.1%和68.4%;联合两种技术诊断的敏感性和特异性分别为95.3%和92.1%。单一技术和联合两种技术诊断的敏感性和特异性之间比较差异均有统计学意义(P<0.05)。结论:联合两种磁共振功能检查技术,可以大大提高对恶性病变的检出能力,但需要结合其他检查,以减少对良性病变的误判。Objective:To investigate the value of weighted imaging Diffusion(DWI) and magnetic resonance spectroscopy(MRS) in differential diagnosis of breast masses.Method:A retrospective analysis of 66 cases of breast nodules or masses were confirmed by pathology.According to the diagnostic criteria,the lesions were classified,and the sensitivity and specificity of single and combined with two kinds of technology in the differential diagnosis of breast carcinoma were observed,the statistical differences were analyzed.Result:A total of 81 breast nodules or masses were found in 66 patients,of which 43 were malignant and 38 were benign.The diagnostic sensitivity and specificity of DWI were 72.1% and 65.8% respectively.The sensitivity and specificity of MRS were 65.1% and 68.4% respectively.The sensitivity and specificity of combined two techniques were 95.3% and 92.1% respectively.There were statistically significant differences between the single technique and the two techniques in the sensitivity and specificity of diagnosis(P〈0.05).Conclusion:Combined with two kinds of magnetic resonance imaging technology can greatly improve the detection ability of malignant lesions,but need to combine with other examination,in order to reduce the false positive lesions.
