摘 要:【目的】探讨缺血预适应训练对缺血性脑血管病患者临床症状改善及预后影响。【方法】选择2012年1月至2014年12月在本院就诊既往有脑梗死病史或短暂性脑缺血发作(TIA)患者200例,随机分为观察组120例,对照组80例,两组均按照脑血管病预防指南用药,观察组进行远程缺血预适应训练,每次5轮,每天2次,连续治疗6个月,分析缺血预适应训练对患者预后影响。【结果】治疗后观察组症状改善率82.50%显著高于对照组67.50%;影像学检查显示,脑血流及代谢改善率85.71%显著高于与对照组48.57%;脑梗死体积缩小率56.67%显著高于对照组32.50%;MRI复检新发脑梗死率4.17%显著低于对照组13.75%。观察组治疗后NIHSS评分显著低于治疗前及对照组,差异均具有统计学意义(P〈0.05);两组患者治疗前后血压水平无显著性差异(P〉0.05)。【结论】缺血预适应训练可显著改善缺血性脑血管病患者神经功能症状及脑血流情况,缩小脑梗死体积,降低脑梗死再发,有利于改善患者预后。[Objective] To investigate the effect of ischemic preconditioning training on ischemic cerebrovascular disease. [Methods]A total of 200 patients with previous ischemic stroke or transient ischemic attack (TIA) from January 2012 to December 2014 were selected. The3/were randomly divided into observation group (120) and control group (80). The patients were treated according to cerebrovascular disease prevention guidelines.-Patients in the observation group were undergoing remote is'chemic preconditioning training, five consecutive rounds / times, 2 times / d, continuous treatment for 6 months. Ischemic preconditioning training on prognosis was analyzed. [Results]The symptomatic improvement rate 82.50 % was significantly higher than that of the control group 67.50%; The imaging examination showed that, cerebral blood flow and metabolism improving 85.71% was significantly higher than that of the control group 48.57 %; infarct volume reduction of the observation group 56.67 % was significantly higher than that of the control group 32.50%, The MRI reviewed new cerebral infarction was 4.17%, which was significantly lower than the control group 13.75% . The NIHSS scores in the observation group after treatment were significantly lower than those before treatment and those of the control group , the differences were statistically significant ( P ~0.05). The blood pressure levels in two groups had no significant difference ( P ~0.05) before and after treatment. [Conclusion] The results show that Ischemic preconditioning training can sig- nificantly improve neurological and cerebral blood flow symptoms in patients with ischemic cerebrovascular disease, reduce infarct volume and infarct recurrence, which will help to improve the prognosis of patients.
