摘 要:目的探讨白介素6(Interleukin6,IL6)和白介素10(Interleukin 10,IL10)在大肠息肉癌病变中的表达变化及临床意义。方法应用荧光定量PCR及免疫组化法分别测量癌旁正常组织、大肠息肉组织、大肠癌组织中IL6和IL10表达量的变化,并进行比较分析。结果 IL6mRNA在腺癌中的相对表达量为2.951±0.474,明显高于正常组织、管状腺瘤、管状绒毛状腺瘤和绒毛状腺瘤组织(其相对表达量分别为1.000±0.000、1.603±0.136、1.660±0.119和1.763±0.143,F=27.309,P<0.05),而后3组差异无统计学意义(F=1.110,P>0.05);癌旁正常组、增生性息肉、锯齿状息肉3组IL6mRNA表达水平无差异(F=1.234,P>0.05),IL6蛋白在腺癌组织组阳性率为72.41%(21/29),与癌旁正常组织10.34%(3/29)、腺瘤性息肉43.24%(32/74)比较差异有统计学意义(χ2=22.916,P<0.05);癌旁正常组织与增生性息肉14.29%(3/21)、锯齿状息肉25.00%(4/16),比较差异无统计学意义(χ2=1.776,P>0.05)。IL10mRNA在腺癌组织中的相对表达量为3.516±0.337,也明显高于正常组织、管状腺瘤、管绒毛状腺瘤、绒毛状腺瘤组织中的表达量(分别为1.000±0.000、1.473±0.134、1.538±0.077、1.577±0.160,F=89.919,P<0.05),并呈现递减趋势;癌旁正常组织、增生性息肉、锯齿状息肉3组IL10mRNA表达水平无差异(F=0.175,P>0.05)。IL10在癌旁正常组织阳性率为13.79%(4/29),明显低于腺瘤性息肉阳性率为44.59%(33/74)和腺癌阳性率为82.76%(24/29,χ2=27.923,P<0.05);癌旁正常组织与增生性息肉14.29%(3/21)和锯齿状息肉18.75%(3/16)比较差异无统计学意义(χ2=0.390,P>0.05)。结论 IL6、IL10可能作用于正常组织-腺瘤性息肉-腺癌的癌变过程,而正常组织-增生性息肉-锯齿状腺瘤癌变过程中无明显作用,提示抑制炎症反应不能阻止组织癌变。Objective To explore the clinical significance of changes in levels of expression interleukin 6(IL6)and interleukin 10(IL10)in the malignant transformation of colorectal polyps. Methods Levels of expression of IL6 and IL10mRNA and protein were measured with fluorescence quantitative PCR and immunohistochemistry,and IL6 and IL10expression were compared in normal tissue,colorectal polyps,and colorectal carcinoma. Results The relative quantity of IL6 mRNA expressed in colorectal cancer(2.951±0.474)was significantly higher than that in normal tissue(1.000±0.000),tubular adenomas(1.603±0.136),tubulovillous adenomas(1.660±0.119),and villous adenomas(1.763±0.143)(F=27.309,P〈0.05).The relative quantity of IL6 mRNA did not differ among normal tissue,hyperplastic polyps,and serrated polyps(F=1.234,P〈0.05).Adenocarcinomas tested positive for IL6 protein at a significantly higher rate(72.41%)than did normal tissue(10.34%)and adenomatous polyps(43.24%)(χ2=22.916,P〈0.05).However,positivity for IL6 protein did not differ among hyperplastic polyps(14.29%),serrated polyps(25.00%),and normal tissue(χ2=1.776,P〈0.05).The relative quantity of IL10 mRNA expressed in colorectal cancer(3.516±0.337)was significantly higher than that in normal tissue(1.000±0.000),tubular adenomas(1.473±0.134),tubulovillous adeno-mas(1.538±0.077),and villous adenomas(1.577±0.160)(F=89.919,P〈0.05).However,the relative quantity of IL10 mRNA did not differ among normal tissue,hyperplastic polyps,and serrated polyps(F=0.175,P〈0.05).Normal tissue tested positive for IL10 protein at a significantly lower rate(13.79%)than did adenomatous polyps(44.59%)and adenocarcinomas(82.76%)(2χ=27.923,P〈0.05).However,positivity for IL10 protein did not differ among hyperplastic polyps(14.29%),serrated polyps(18.75%),and normal tissue(2χ=0.390,P〈0.05).
