摘  要:目的应用点波谱分辨技术(PRESS)进行后扣带回(PCC)波谱成像,探索阿尔茨海默病(AD)患者特定脑区化合物代谢水平。方法以MMSE和CDR评分为标准,纳入本院老干部AD患者及健康对照组各20例。选用T1WI-3D图像横断位、矢状位及冠状位3平面进行PCC波谱定位,采用PRESS,单体素波谱(SVS-SE)序列行PCC波谱采集,分析N-乙酰天门冬氨酸(N-acetylaspartate,NAA)、胆碱(cholines,Cho)、肌醇(myo-inositol,mI)和肌酸(creatine,Cr)水平及NAA/Cr、mI/Cr、NAA/mI和Cho/Cr之间的相对比值。结果波谱分析发现,AD组NAA峰较对照组不同程度减低,m I峰较对照组升高。AD组NAA/Cr较正常对照组减低(t=-2.502、P=0.017),NAA/mI较正常对照组也降低(t=-9.16、P=0),mI/Cr较正常对照组升高(t=9.78、P=0),Cho/Cr较正常对照组升高(t=1.76、P=0.086)。mI/Cr比值与其MMSE评分呈负相关(r=-0.648,P=0.002)。结论磁共振波谱技术研究扣带回有助于AD的筛查及早期诊断。Objective To study metabolism levels in specific regions of the brain of Alzheimer's disease(AD) patients by generating posterior cingulate cortex(PCC) spectroscopy with point-resolved selective spectroscopy(PRESS) Methods Twenty patients with Alzheimer's disease were selected into AD group and another 20 into control group based on MMSE and CDR scores.T1WI-3D images in transverse,sagittal and coronal plane were selected for posterior cingulate spectrum positioning; PRES and single voxel spectroscopy(SVS) sequence were used for PCC spectrum acquisition. Levels of N-acetyl aspartic acid(NAA),choline(Cho),myo inositol(mI),and creatine(Cr) and the relative ratios between NAA/Cr,mI/Cr,NAA/mI and Cho/Cr levels were analyzed.Results Spectrum analysis showed that NAA peak levels for patients in AD group were lower by different degrees compared with those in control group, while mI peak levels of patients in AD group were higher than those in control group. Compared with control group, AD group had lower NAA/ Cr ratios(t =-2.502,p = 0.017),lower NAA/mI ratios(t =-9.16,P = 0), higher mI/Cr ratios(t =9.78,P = 0), and higher Cho/Cr ratios(t = 1.76,P = 0.086). MI/Cr ratios were negatively correlated with MMSE score(r =-0.648,P= 0.002). Conclusion The use of magnetic resonance spectroscopy in studying PCC of patients with Alzheierm's disease can be helpful for the screening and early diagnosis of Alzheimer's disease.
