摘　　要：目的分析老年性白内障患者住院费用构成、变化趋势及影响因素,为合理控制住院费用提供依据。方法汇总海南省某医院信息系统中2010年-2014年间2522位老年性白内障住院患者病案资料。利用Excel、Visual Fox Pro 6.0与SPSS16.0统计软件整理并分析数据。采用Wilcoxon秩和检验、Kruskal-Wallis H检验行单因素分析,应用多重线性回归方法行多因素分析。结果女性患者构成比略高于男性,分别为51.51%与48.49%。70岁以上组患者构成比最大,占37.99%。药品费、手术费占住院费用构成比虽逐年下降,但仍大于15%。治疗费逐年上升,2014年高达45.97%。多因素分析显示,性别、年龄、住院天数、是否手术、是否转科、年份为住院费用影响因素。结论合理控制药品费、手术费、治疗费、住院天数、转科有利于降低住院费用。Objectives To analyze the constitution, changing tendency and the influencing factors of hospitalization expense in patients with senile cataract, and provide references for appropriate control of hospital expense. Methods To collect medical record information of 2522 inpatients with senile cataract between 2010 and 2014 from a hospital information system in Hainan Province. Then statistical software such as Excel, Visual FoxPro 6. 0 and SPSSl6. 0 were used to process data. Wilcoxon rank sum test and Kruskal-WaUis//test were adopted for single factor analysis, while the method of multiple linear regression was applied for multiple factor analysis. Results Constituent ratio in female patients was slightly larger than male, being 48.49%, 51.51% respectively. The group of inpatients whose age were between 70 and 79 makes up as large as 37. 99% of the total amount, lager than any other group. Constituent ratios in costs for both drugs and surgery present a declining trend year by year, but the proportion was still greater than 15%. On the contrary, the ratio for treatment expense had increased, being up to 45.97% in 2014. Multiple factor analysis shows that gender, age, length of hospital stay, surgery, transferring and year were the influencing factors in expense, with P〈0. 05. Conclusions Reasonable control of drug costs, operation fees and treatment costs, as well as length of hospital stay and transferring could be helpful to reduce the hospitalization expense.
