北京某医院宫颈高危人乳头瘤病毒亚型感染情况调查

摘　　要：目的观察北京某医院宫颈上皮细胞高危人乳头瘤病毒（high risk human papiloma virus,HR-HPV）亚型的感染情况。方法对2012-09至2015-03在我院门诊患者,采用HPV分型检测的方法进行机会性筛查,观察13种HR-HPV在人群中的感染率及不同年龄段人群的差别。结果 4654例受检人群中HPV总感染率18.59%（865/4654）,其中单一亚型HPV感染和两种以上HPV亚型多重感染分别占阳性总数的74.34%（643/865）和25.66%（222/865）。除去单一低危亚型HPV感染67例（1.44%,67/4654）,HR-HPV的总感染率为17.15%（798/4654）。感染率排在前6位的HR-HPV亚型分别为HPV16（3.59%）、HPV52（3.31%）、HPV58（3.24%）、HPV51（2.41%）、HPV39（2.30%）和HPV18（1.78%）。20岁之前有性生活的女性HR-HPV的感染率最高,达30.05%,此后随年龄增长呈下降趋势;40岁后再度上升,46～50岁可达16.55%,50岁后降至最低为12.47%。结论北京某医院常见的HR-HPV亚型为HPV16、HPV52、HPV58,在20岁前和46～50岁呈两个峰值,值得临床关注。Objective To investigate the infection of high risk human papillomavirus subtype in cervical epithelium in Beijing. Methods From September 2012 to March 2015,female outpatients in this hospital received HPV subtype gene testing. The infection rate of 13 kinds of HR-HPV in the crowd and in different age group were observed. Results The total infection rate of HPV was 18. 59%（ 865 /4654） in this group,among these the rate of single subtype HPV infection was 74. 34%（ 643 /865）,the multiple subtype infection accounted for 25. 66%（ 222 /865）. The total infection rate of HR-HPV was 17. 15%（ 798 /4654）,except 67 cases of single low risk HPV infection. The top six subtypes of HR-HPV in crowd were HPV16（ 3. 59%）,HPV52（ 3. 31%）,HPV58（ 3. 24%）,HPV51（ 2. 41%）,HPV39（ 2. 30%）,and HPV18（ 1. 78%）. Before the age of 20 the infection rate of HR-HPV was the highest（ 30. 05%）,and then decreased with the increase of age. The infection rate increased after the age of 40 and during 46-50 a second peak was observed（ 16. 55%）,after the age of 50 the lowest infection rate（ 12. 47%） was observed. Conclusion The common HR-HPV subtypes in Beijing region are HPV16,HPV52 and HPV58,and the age distribution manifests two peaks.
