摘 要:目的探讨应用高频超声检查对孤立性小腿肌间静脉血栓进行诊断的超声特点及其临床应用价值。方法对2013年2月至2015年7月期间高度怀疑孤立性小腿肌间静脉血栓126例患者进行高频超声检查,依据不同的血栓形成部位,将高频超声诊断正确的92例患者分为腓肠肌间静脉组(A组,43例),比目鱼肌间静脉组(B组,49例),并对两组的临床资料以及高频超声参数进行比较分析。结果B组的静脉宽度明显宽于A组,差异有统计学意义(P〈0.05);B组的血栓长度明显长于A组,差异有统计学意义(P〈0.05);经抗凝溶栓治疗后,B组的静脉再通率明显低于A组,差异有统计学意义(P〈0.05);高频超声检查诊断孤立性小腿肌间静脉血栓的敏感度,特异度及准确率分N96.84%、93.55%、96.03%。结论高频超声检查能无创、准确地诊断孤立性小腿肌间静脉血栓并可对其进行实时动态疗效监测,有着重要的临床意义。Objective To explore the imagingcharacteristics and the and the clinical application value of high frequency ultrasound in isolated calf muscular venous thrombosis ( ICMVT ) diagnosis. Methods In February 2013 to July 2015, due to the highly suspected ICMVT in our hospital, 126 patients with high frequency ultrasound. Based on different parts of thrombosis, 92 patients of high frequency ultrasound diagnosis right were divided into Gastrocnemius muscle group ( group A, 43 cases ) and the soleus group ( group B, 49 cases ) , and the clinical data and high frequency ultrasound parameters were compared. Results The vein width of B group was obviously wider than that in group A, the significant difference ( P〈0.05 ) ; Thrombus length of B group was obviously longer than that in group A, the significant difference ( P〈0.05 ) ; After anticoagulant thrombolytic therapy, venous patency rate of group B was obviously lower than that in group A, the significant difference ( P〈0.05 ) ; The sensitivity, specificity and accuracy of high frequency ultrasound in ICMVT diagnosis were 96.84%, 93.55%, 96.03%. Conelusion High frequency ultrasound can make ICMVT accurately and noninvasive diagnosis, can carry on the real-time dynamic monitoring, has important clinical significance.
