摘  要:目的探讨高亲和力Ig E受体(FcεRI)基因启动子区多态性(MS4A2 rs1441586和FcεRIG rs11587213)与青霉素过敏的相关性。方法以甘肃地区汉族人群200例青霉素过敏患者和210例对照为研究对象,采用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)技术检测MS4A2 rs1441586和FcεRIG rs11587213两位点基因型及等位基因的分布频率,并通过测序进行验证。结果过敏组MS4A2 rs1441586位点基因型频率与对照组相比差异无统计学意义(χ^2=5.497,P〉0.05),过敏组C等位基因频率为35.8%,高于对照组的29.3%(χ^2=3.906,P〈0.05);按性别分组后其基因型及等位基因频率分布差异无统计学意义;按年龄分组后,非老年人群中过敏组基因型分布频率与对照组相比差异有统计学意义(χ^2=5.908,P〈0.05)。过敏组FcεRIG rs11587213位点基因型频率与对照组相比差异无统计学意义(χ^2=5.526,P〉0.05),过敏组G等位基因频率为7%,高于对照组的3.6%(χ^2=4.847,P〈0.05);按性别分组后,男性过敏组与对照组基因型及等位基因频率分布差异仍有统计学意义(χ^2分别为8.649、4.294,P均〈0.05);按年龄分组后,老年人群中两组间差异有统计学意义(χ^2分别为7.637、5.550,P均〈0.05)。结论 FcεRI基因启动子区多态性可能与甘肃地区汉族人群青霉素过敏反应相关,MS4A2 rs1441586与非老年人青霉素过敏高度关联,FcεRIG rs11587213与男性以及老年青霉素过敏患者高度关联。Objective To investigate the correlation of high-affinity Ig E receptor( FcεRI) promoter region polymorphisms( MS4A2rs1441586 and FcεRIG rs11587213) with penicillin allergy. Methods Two hundreds of patients with penicillin allergy and 210 healthy controls from the Han population of Gansu province were enrolled in the study. Their genotype and allele frequency distributions of MS4A2 rs1441586 and FcεRIG rs11587213 were determined by polymerase chain reaction-restriction fragment length polymorphism( PCR-RFLP),and the results were further verified by sequencing. Results There was no significant difference in the genotype frequency of MS4A2 rs1441586 between the patients with penicillin allergy and healthy controls( χ^2= 5. 497,P 〉0. 05),however,the C allele frequency( 35. 8%) in the former was significantly higher than that in the latter( 29. 3%,χ^2= 3. 906,P〈 0. 05). When stratified by gender,there was no significant difference in the genotype and allele frequency of MS4A2 rs1441586 between the two groups,however,when stratified by age,there was significant difference in the genotype frequency of non-aged populations between the two groups( χ^2= 5. 908,P〈 0. 05). There was no significant difference in the genotype frequency of FcεRIG rs11587213 between the patients with penicillin allergy and healthy controls( χ^2= 5. 526,P〉 0. 05),however,the G allele frequency in the former( 7%) was significantly higher than that in the latter( 3. 6%,χ^2= 4. 847,P〈 0. 05). When stratified by gender,there was significant difference in the genotype and allele frequency of males between the two groups( χ^2= 8. 649 and 4. 294,P 〈0. 05),while,when stratified by age,there was significant difference in the genotype and allele frequency of the aged between the two groups( χ^2= 7. 637 and 5. 550,P〈0. 05).
