摘 要:目的:建立患者特征性下腔静脉闭塞型布-加综合征( BCS )三维模型,分析血流动力学参数变化,探讨其在临床诊疗中的意义。方法选取下腔静脉闭塞型BCS病例,通过对MR影像数据的三维重建,采用计算流体动力学( CFD)方法进行血流动力学数值模拟,分析其主要血流动力学参数变化。结果建立的闭塞型下腔静脉三维流体模型揭示了病变区域的血流模式。该病例中:下腔静脉血管闭塞段血流速度较慢,在侧支血管处血流速度加快;下腔静脉血管闭塞段静压力较大,在侧支血管处静压力下降;闭塞段下腔静脉壁面切应力较大,到达侧支血管之后切应力值显著下降。结论血流模式的改变影响血管内血液的局部流速以及血管壁上的切应力等参数分布,这些可能是造成下腔静脉闭塞型BCS下腔静脉增粗及血栓形成的因素。Objective To establish a three-dimensional simulation model of Budd -Chiari syndrome ( BCS ) with occlusion of the inferior vena cava in order to analyze the variation in hemodynamic parameters and discuss its clinical sig -nificance.Methods A three-dimensional model was established using MRI images from a BCS patient with occlusion of the inferior vena cava .Then, numerical simulation was conducted through computational fluid dynamic ( CFD) method to analyze the variation in major hemodynamic parameters .Results The established model showed in the blood flow pat-tern of damaged areas , where slow blood flow and high static pressure were detected in the inferior vena cava , in compari-son with fast blood flow and low static pressure in collateral vessels .The shear stress was high in the inferior vena cava wall but remarkably decreased in collateral vessels .Conclusion The alteration in the blood flow pattern can affect pa-rameters including local intravascular blood flow rate and vascular wall shear stress , which might contribute to enlarge-ment of the inferior vena cava and consequent thrombosis in BCS patients .
