后腹腔镜下脂肪囊外肾蒂淋巴管结扎术的护理观察

摘　　要：目的：比较后腹腔镜下游离脂肪行肾蒂淋巴管结扎术与脂肪囊外肾蒂淋巴管结扎术治疗乳糜尿患者的术后恢复情况和护理特点。方法选择后腹腔镜下脂肪囊外肾蒂淋巴管结扎术治疗的乳糜尿患者20例（ A组），后腹腔镜下游离脂肪肾蒂淋巴管结扎术治疗的乳糜尿患者20例（B组），观察比较2组患者术后恢复情况。结果 A组患者术中出血量、术后下床时间、胃肠功能恢复时间、住院时间均短于B组，抑郁自评量表（ SDS ）评分、焦虑自评量表（SAS）评分低于B组，差异均有统计学意义（P＜0．05）。结论后腹腔镜下脂肪囊外肾蒂淋巴管结扎术较游离脂肪肾蒂淋巴管结扎术具有创伤小、痛苦少、恢复快等优点。Objective To compare the efficacies and nursing characteristics between retroperitoneoscopic renal ped -icle lymphatic disconnection in free fatty and retroperitoneoscopic renal pedicle lymphatic disconnection outside adipose capsules for treatment of chyluria .Methods A total of 40 chyluria patients were divided into two groups （n=20）, who underwent retroperitoneoscopic renal pedicle lymphatic disconnection in free fatty （Group A） and retroperitoneoscopic re-nal pedicle lymphatic disconnection outside adipose capsules （ Group B） , and their nursing care was compared .Results Compared with patients in Group B , those in group A showed remarkably less blood loss during surgery , and shorter out-of-bed time and hospital stay , and faster gastrointestinal recovery , as well as lower scores of Self -Rating Depres-sion Scale （SDS） and Self-Rating Anxiety Scale （SAS） （P〈0.05）.Conclusion Retroperitoneoscopic renal pedicle lymphatic disconnection outside adipose capsules has small incision , less pain, and quick recovery .
