无浆膜层侵犯的进展期胃癌行腹腔镜辅助远端胃癌D2根治术的治疗效果

摘　　要：目的探究无浆膜层侵犯的进展期胃癌行腹腔镜辅助远端胃癌D2根治术治疗的临床疗效。方法对2012年1月至2014年7月在接受手术治疗80例无浆膜层侵犯胃癌患者的临床资料进行回顾性分析,依据所采用手术方法的不同分为研究组(腹腔镜辅助手术)和对照组(开腹手术),每组40例。对比分析两组患者术中、术后指标,并发症以及生存情况。结果研究组的手术用时较对照组长,而在切口长度、术中出血量和术后胃肠道功能恢复时间以及住院时间上确较对照组具有优势(P<0.05);在淋巴结清扫数量、并发症发生率和无瘤生存率上两组间差异无显著性(P>0.05)。结论无浆膜层侵犯展期胃癌采用腹腔镜辅助D2根治术安全、可行,具有创伤小、切口相对美观、恢复快等优势,在淋巴结清扫上与开腹手术具有相同的效果,值得临床应用。Objective To investigate the effect,feasibility and safety of laparoscopic- assisted distal gastric cancer of D2 radical gastrectomy in treatment of advanced gastric cancer patients without serosal invasion. Methods A total of 80 advanced gastric cancer inpatients without serosal invasion during January 2012 and July 2014 were recruited in this study,and were divided into research group（ n = 40） and control group（ n = 40） according to the operative methods. The research group underwent laparoscopic- assisted distal gastric cancer of D2 radical gastrectomy,while the control group underwent open radical operation. Comparison of the surgical effect- related indicators between 2 groups was performed.Results In the research group,the operation time was longer,intraoperative bleeding volume was less,length of incision shorter,length of stay shorter,and postoperative recovery time of gastrointestinal function was shorter than those in the control group（ P〈0. 05）. The differences in number of lymph node dissection,the incidence rate of complications and tumor free survival rate were not statistically significant in the two groups（ P〉0. 05）. Conclusion Laparoscopic- assisted distal gastric cancer of D2 radical gastrectomy in treatment of advanced gastric cancer patients without serosal invasion is effective and safe. Advantage have less invasive,relatively beautiful incision,and quicker recovery. In the lymph node dissection and open surgery has the same effect,it is worthy of clinical application.
