摘 要:目的探讨子宫颈腺癌高危型人乳头状瘤病毒(HR-HPV)感染检测及p16INK4A蛋白表达的临床意义。方法选取25例宫颈上皮内瘤变CINⅠ、30例CINⅡ、35例CINⅢ、24例浸润性腺癌,同时选取35例正常宫颈组织(慢性炎症)患者。采用免疫组织化学方法检测组织中p16INK4A蛋白表达情况,并应用第二代杂交捕获法(HC-2)对13种HR-HPV DNA进行检测。结果宫颈上皮内瘤变CINⅠ、CINⅡ、CINⅢ组织、浸润性腺癌HR-HPV感染阳性率和p16INK4A蛋白表达阳性率均显著高于正常宫颈细胞(P<0.05);宫颈组织中HR-HPV感染阳性率与p16INK4A蛋白表达为显著正相关性(P<0.05)。结论高危型人乳头状瘤病毒(HR-HPV)导致的宫颈恶化转化与p16INK4A蛋白表达密切相关,采用p16INK4A与HR-HPV联合诊断有利于提高子宫颈癌及CIN阳性预测值。Objective To explore detection of high-risk human papilloma virus( HR-HPV)infection and expression of p16INK4 A protein in patients with cervical adenocarcinoma. Methods Twenty five cases of cervical intraepithelial neoplasia CIN Ⅰ,30 cases of CIN Ⅱ,35 cases of CINⅢ,and 24 cases of infiltrating adenocarcinoma as well as 35 cases of normal cervical tissue( with chronic inflammation) were selected and detected by immunohistochemical method for expression of p16INK4 A protein and the second generation of hybrid capture technique( HC-2) for DNA in 13 types of HR-HPV. Results The positive infectious rates of HR-HPV and positive expression rates of p16INK4 A protein in cervical intraepithelial neoplasia CIN Ⅰ,CIN Ⅱ,CIN Ⅲ tissues and infiltrating adenocarcinoma tissues were significantly higher than those in the normal cervical cells( P〈0. 05). Positive rate of HR-HPV infection was positively correlated with p16INK4 A protein expression in cervical tissues( P〈0. 05). Conclusion Cervical deterioration caused by HR-HPV is closely related with p16INK4 A protein expression,and the combined diagnosis of p16INK4 A protein and HRHPV can improve the positive predictive values of cervical cancer and CIN.
