摘 要:目的探讨乳腺癌间质中CD68^+和CD206^+肿瘤相关巨噬细胞(tumor associated macrophages,TAMs)的浸润与临床病理特征的关系及其对预后的影响。方法应用免疫组化Max Vision法检测172例乳腺癌和50例乳腺良性病变组织中CD68和CD206的表达,比较CD68^+TAMs和CD68^+/CD206^+TAMs浸润密度与乳腺癌临床病理特征的关系及其对预后的影响。结果CD68^+TAMs和CD68^+/CD206^+TAMs在乳腺癌的浸润密度均较良性病变组织升高(P均<0.000 1);CD68^+/CD206^+TAMs高密度表达分别与肿瘤直径增大、淋巴结分期及临床分期增高等有关(P<0.000 1,P=0.007,P<0.000 1)。CD68^+/CD206^+TAMs高密度浸润组患者的无病生存率及总生存率均较低密度浸润组降低(P=0.013,P=0.003)。结论 CD68^+TAMs和CD68^+/CD206^+TAMs在乳腺癌组织中有较高的浸润密度,CD68^+/CD206^+TAMs可能与乳腺癌的发生、发展密切相关,并有望成为预测乳腺癌预后的重要潜在标志物。Purpose To examine the infiltration of CD68 positive and CD206 positive tumor associated macrophages (TAMs) in breast cancer tissue and their relationship with clinicopathological characteristics and prognosis. Methods MaxVision immunohistochemistry method was used for the staining of CD68 and CD206 in paraffin-embedded tissue sections from 172 breast cancer cases and 50 cases of benign breast lesions, the relationship between different infiltration density of CD68 + TAMs and CD68 +/CD206 +TAMs with clinico- pathological characteristics and prognosis was analyzed. Results The average invasive density of CD68 + TAMs and CD68 +/CD206 +TAMs were significantly higher in breast cancer tissue than this in the benign breast lesions (P 〈 0. 000 1, P 〈 0. 000 1 ). The higher invasive density of CD68 +/CD206 + TAMs was correlated to larger tumor size, higher lymph node stage and higher TNM stage ( P 〈 0. 000 1, P = 0. 007, P 〈 0. 000 1 ). In addition, higher-density infiltration of CD68 +/CD206 + TAMs were correlated to poor disease free survival (DFS) and poor overall survival (OS) (P =0.013, P =0.003). Conclusion TAMs with both CD68+ and CD206+ have higher-density infiltration in breast cancer tissue, and CD206 + TAMs might be closely related with breast cancer progression. CD68 +/CD206 + TAMs may be used to predict breast cancer prognosis.
