摘　　要：目的探讨胃癌组织中程序性死亡蛋白-1(programmed death-1,PD-1)及其配体-1(programmed death ligand-1,PD-L1)表达与胃癌转移或复发的关系。方法选取本院2005年9月至2010年9月行胃癌根治术并经病理确诊的患者200例为研究对象。采用SP法检测PD-1及PDL-1在胃癌组织中的表达水平,采用Cox回归模型探讨PD-1及PDL-1表达与肿瘤复发或转移的关系。结果胃癌组织中PD-1表达与患者性别、年龄、术前癌胚抗原(CEA)水平、肿瘤N分期无明显关系,但与肿瘤淋巴结转移率(lymph node ratio,LNR)密切相关;胃癌组织中PD-L1表达与患者术前CEA水平、肿瘤N分期、LNR密切相关。LNR(P=0.013)、PD-L1阳性(P=0.002)、肿瘤浸润深度(P=0.05)是胃癌转移或复发的独立危险因素。结论胃癌组织中PD-L1表达阳性与患者术前CEA水平、肿瘤N分期、LNR密切相关,是胃癌转移或复发的独立危险因素。Objective To discuss the relationship between the expression of programmed death-1（PD-1） and its ligand programmed death ligand-1（PD-L1） in gastric cancer tissues and the prognostic in gastric cancer. Method Retrospective analyzed the clinical date of 200 patients with gastric cancer from September 2005 to September 2010, mutivaritate analysis was performed used Cox proportional hazard regression model stepwise. Result The expression of PD-L1 in gastric cancer tissues had nothing to do with sexuality, age, preoperative carcinoembryonic antigen（CEA） level, tumor N stage, it was only closely related to LNR; the expression of PD-L1 in gastric cancer tissues was closely related to preoperative CEA level, tumor N stage, and LNR. LNR（P = 0.013）, PD-L1 positive（P = 0.002） and tumor invasion depth（P = 0.05） were the independent risk factors for metastasis or recrudescence of gastric cancer. Conclusion PD-L1 positive is associated with preoperative CEA level, tumor N stage, LNR, and PD-L1 is the independent risk factors of metastasis or recrudescence.
