联合筋膜鞘悬吊术治疗提上睑肌缩短术后复发中重度上睑下垂

摘　　要：目的探讨提上睑肌缩短术治疗上睑下垂手术失败后,利用联合筋膜鞘悬吊再次手术矫正的效果。方法 2008年12月-2014年12月6年间,利用该技术治疗提上睑肌缩短术后复发中重度上睑下垂225例346只眼,单侧104例,双侧121例,在局部麻醉下将睑板缝合悬吊到位于结膜上穹窿的上直肌与提上睑肌之间的联合筋膜鞘上,可以有效地提高上眼睑的高度,术后切口均一期愈合。结果随访1年到3年,其中满意273只眼,占79%,改善63只眼,占18%,无效10只眼,占3%。对10只无效眼及63只改善眼做了二次调整,调整时间最短为术后3周,最长为术后6个月,调整后除6只眼为改善,其余均达到满意标准。双睑睑裂对称,重睑弧度自然,无睑裂闭合不全加重。结论应用结膜上穹窿联合筋膜鞘悬吊技术治疗提上睑肌缩短术后复发性中重度上睑下垂,既能有效地提高上眼睑高度,又能获得很好的美容效果。Objective To explore ptosis surgery after failure by levator palpebrae superioris shortening, and evaluate the clinical result of reoperation correction again by using conjoint fascial sheath（CFS） suspension. Method： Since Dec. 2008 to Dec. 2014, 225 moderate or severe blepharoptosis patients（346 eyelids, unilateral in 104 patients, bilateral in 121 patients）, all the eyelids were operated before by levator plapebrae superioris shortening, under local anesthesia,all cases used CFS suspension technology, the CFS located between levator and superior rectus, is brought forward and sutured to the tarsus, which raises the eyelid, postoperative incision were primary healing. Results： All cases were followed up 1 year to 3 years, the eyelids were raised with 79%（273） normalization, 18%（63） improvement, and only 3%（10） eyelid showed no changed. To achieve a good final result, these 73 eyelids were revised once between 3 weeks to 6 months, 6 eyelids improvement, others are to a good final result after reoperated, the palpebral fissure of two eyes were symmetrical, dual eyelid was better and there were no more severe layophthalmos occured. Conclution： The CFS suspension technique is an effective method of treating recurrent blepharoptosis, the advantages with this technology are to be quite successful in elevating motion of the physiologic eyelids, and also improve the cosmetic appearance.
