摘　　要：目的评价不同年龄对阿替普酶静脉溶栓治疗急性脑梗死的临床疗效和预后的影响。方法将98例脑梗死患者按年龄分为高龄组52例和低龄组46例。2组患者均予以0.9 mg·kg-1阿替普酶静脉溶栓治疗。比较2组患者溶栓前和溶栓后1,21 d的临床疗效,美国国立卫生研究院卒中量表(NIHSS)评分以及不良反应发生情况,并于3周后进行改良Rankin量表(mRS)评分。结果治疗后,2组患者的NIHSS评分均明显低于治疗前(P<0.05)。治疗后1,21 d,低龄组的NIHSS评分明显低于高龄组(P<0.05)。治疗后,低龄组的不良反应发生率明显低于高龄组(P<0.05)。治疗3周后mRS评分表明,低龄组的预后明显优于高龄组(P<0.05)。结论阿替普酶静脉溶栓治疗急性脑梗死,2组患者均可以从治疗中获益,但低龄组的溶栓效果明显优于高龄组,且并发症较少,预后相对较好。Objective To analysis the effect of different age factor on the prognosis and efficacy of acute cerebral infraction in patients treated by intravenous thrombolytic therapy with alteplase. Methods Ninety-eight patients with cerebral infarction were divided into older group（ n = 52） and younger group（ n = 46）. Patients in two groups were treated by intravenous thrombolysis with 0. 9 mg · kg-1alteplase. After 1and 21 d,the efficacy was observed and scored using national institute of health stroke scale（ NIHSS）. At the same time,the incidence of adverse drug reactions were observed,and prognosis evaluated using Modified Rankin scale（ mRS） in 3 weeks. Results NIHSS scores were significantly lower in the both groups treated by alteplase than those before therapy（ P〈0. 05）. In 1 and 21 d,NIHSS scores were lower in the younger groups than those in elderly group（ P〈0. 05）. The incident of adverse drug reaction was lower in the younger groups than the elderly group（ P〈0. 05）. It has shown that the prognosis was better in the younger group than the elderly group（ P〈0. 05） by comparing mRS score in 3weeks. Conclusion All acute cerebral infraction patients received significant benefits from the intravenous thrombolytic therapy with alteplase.The younger group had more significant efficacy,less complications and better prognosis than those of elderly group.
