美学区种植修复的十年临床回顾性研究

摘　　要：目的本研究统计了美学区种植牙修复的长期存留率,分析了不同的影响因素对美学区生存率的影响及其作用。方法回顾性统计西安交通大学口腔医院种植科自2003年11月至2014年12月美学区种植修复患者的临床信息,定期规律随访,用寿命表法统计累计存留率,用Ka Plan-Meier法和Cox回归模型分析不同因素对生存率的影响。结果 2003年11月至2013年12月共有452名患者接受了美学区种植体植入术,其中男性305名,女性147名,共计774枚种植体。通过寿命表法计算得到3、5、7年的累积生存率分别为98.9%、98.6%、98.1%。种植体周围状态、种植体系统、中央螺丝状态、牙周炎这四个因素影响种植体的存留率。多因素分析显示,种植体周围探诊出血、Branemark种植系统、中央螺丝异常与种植失败明显相关。结论基于10年临床资料的分析,证明纯钛种植体修复美学区缺牙是一种长期稳定可靠的修复方式,10年累积生存率超过95%。多因素分析显示,对可能的混杂因素进行矫正后,种植体周围探诊出血、机械抛光的Branemark种植系统、中央螺丝异常可以预测种植失败的发生。Objective By analyzing the long-term survival in the esthetic zone, we aim to explore the impact of different factors on implants＂ survival and make some advice for clinical practice.Methods Information of patients who received implants in the esthetic zone in our implant center from November 2003 to December 2014 was retrospectively reviewed. Patients were evaluated after 3 months,6 months,and every 1 year interval by means of regular in-patient check and telephone.The cumulative survival rate was calculated using the life table method.The impact of different factors on implants＂ survival was analyzed by Kaplan Meier method or Cox regression model.Variables with P-value 〈 0.05 on two-sides were considered statistically significant.Results During the period, a total of 774 implants were performed in 452 patients, including 305 males and 147 females. The cumulative survival rate at the implant level was found to be 98.9%, 98.6%, and 98.1%, at 3, 5 and 7 years mark, respectively. The nnivariate analysisidentified a relationship between the following risk indicators for implant failure：implant system, status of central screw, periodontitis and peri-implantitis.After adjusted by possible confounders,association was observed between implant failure and implant type of branemark, abnormal status of central screw, peri-implantitis.Conclusion Based on the ten-year experience in our solitary center,pure titanium implants is a stable way for teeth restoration in esthetic zone with a favorable survival rate more than 95%.Factors including peri-implantitis and could predict the rare occurrence of implant failure.
