摘 要:目的应用可视化角膜生物力学分析仪(Corvis ST)评估准分子激光原位角膜磨镶术(LASIK)术后远期角膜生物力学变化。方法回顾性病例研究。收集1996年12月至2008年12月在西安市第四医院眼科70例(140只眼)LASIK手术后远期患者(6~17年)进行角膜生物力学测量,平均随访年限(8.68±3.74)年。同时选取该院眼科激光视力矫正中心行手术前检查并行飞秒激光制瓣LASIK手术患者63例(126只眼),手术前及手术后早期1~3月,平均随访时间(1.73±0.75)个月作为未手术对照组及手术后早期对照组。应用CorvisST可视化角膜生物力学分析仪可获得第一次压平长度(AL1)、第一次压平速度(CV1)、第二次压平长度(AL2)、第二次压平速度(CV2)、第一次压平时间(AT1)、第二次压平时间(AT2)、最大压陷深度(Def.Ampl)、最大屈膝蜂间距(P.Dist)和最大压陷曲率半径(HCRadius)等角膜生物力学参数,分析比较LASIK手术前后角膜生物力学参数变化特点;并对比分析手术后早期组、手术后远期组角膜生物力学情况。结果与未手术角膜组对比,FS—LASIK术后早期、LASIK术后远期生物力学各项指标差异均有统计学意义,其中CV1(P=0.00),AL2(P=0.00),AT1(F=36.36,P=0.00),HCRadius(F=104.18,P=0.00)较未手术角膜组降低,差异有统计学意义;CV2绝对值(F=29.35,P=0.00),AT2(F=10.29,P=0.00),Def.Ampl(F=26.25,P=0.00),P.Dist(P=0.00)较未手术角膜组增加,差异有统计学意义。FS—LASIK术后早期、LASIK术后远期两组对比,AL1(Z=-2.64,P=0.01)差异有统计学意义外、CV1(Z=-0.14,P=0.89);AL2(Z=-0.71,P=0.48);CV2(t=1.26,P=0.21);AT1(t=-0.57,P=0.57);AT2(t=-1.48,P=0.14);Def.Ampl(t=1.43,P=0.16);P.Dist(Z=-1.08,P=0.28);HCRadiObjective To evaluate long-term corneal biomechanical properties after LASIK by Scheimpflug-based dynamic tonometry (Corvis ST). Methods In this retrospective study, 70 patients (140 eyes) who underwent LASIK more than six years previously at Xi'an No.4 Hospital were en- rolled in this study as the postoperative long term LASIK group, 63 patients (126 eyes) myopic eyes without any ophthalmologic surgery and the 63 patients (126 eyes) treated by femtosecond la- ser assisted LASIK (FS-LASIK) as group, respectively. Changes in the preoperative control group and corneal deformation parameters, postoperative short time control including deformation amplitude (DA), applanation time (AT1 and AT2), applanation length (ALl and AL2), corneal velocity (CV1 and CV2), peak distance (P.Dist), HC Radius at highest concavity (HC Radius) values were mea- sured in all eyes with the Corvis Scheimpfiug Technology (Corvis ST, OCULUS, Wetzlar, Germany). Results A significant difference was detected among the three groups. Corvis ST showed significant differences in DA (F =26.25, P =0.00), ATI(F =36.36, P =0.00) and AT2 (F =10.29, P =0.00), ALl (P =0.00) and AL2 (P =0.00), CV1 (P =0.00) and CV2 (F =29.35, P =0.00), EDist. (P =0.00), HC Radius (F =104.18, P =0.00) values among groups. Compared to normal control group, the CV1, AL2, ATland HC Radius were significant lower; the CV2, AT2, DA and EDist. were significant higher in postoperative long term LASIK group and postoperative short time FS-LASIK group. Al- though a significant difference was observed in ALl between the postoperative long term LASIK group and postoperative short time FS-LASIK group, Corvis ST showed no differences in DA (t = 1.43, P =0.16), AT1 (t =-0.57, P =0.57) and AT2 (t =-1.48, P =0.14), ALl (Z =-2.64, P =0.01) and AL2 (Z =-0.71, P =0.48), CV1 (Z =-0.14, P =0.89) and CV2 (t =1.26, P =0.21), P.Dist (Z =-1.08, P =0.28), HC Radius (t =1.97, P =0.06
