术中预防性截囊技术对糖尿病患者白内障术后囊袋收缩的影响

摘　　要：目的探讨术中预防性截囊技术对糖尿病患者白内障术后囊袋收缩的预防效果。方法临床病例对照研究。对2013年7月至2014年7月间在安阳市眼科医院行白内障超声乳化吸除联合人工晶状体植入术的合并糖尿病的白内障患者93例（93只眼），其中实验组45例、对照组48例，实验组术中人工晶状体植入后给予预防性截囊，对照组则否。术后观察6个月，比较两组前房炎症反应、前囊膜混浊程度、撕囊口直径的改变、人工晶状体位置的变化、后发性白内障的发生等情况。结果实验组与对照组术后第1d及第1周的前房炎症反应比较，差异没有统计学意义（P〉0．05）；两组患者术后第6个月的前囊口收缩情况比较，差异具有统计学意义（P〈0．01），对照组有5例出现了3级前囊口收缩。所有患者术后人正晶状体位置均未发生变化，后发性白内障的发生在两组之间差异也没有统计学意义。结论术中植入人工晶状体后行预防性截囊技术，可以有效预防糖尿病患者白内障术后囊袋收缩的发生，同时并不增加后发性白内障等并发症的发生。Objective To examine the effect of preventative capsulotomy on prevention of ante- rior capsule contraction after cataract surgery in patients with diabetes mellitus. Methods Nine- ty-three patients with diabetes mellitus who underwent cataract surgery between July 2013 and July 2014 in Anyang Eye Hospital were divided into two groups. The experimental group （n=45） had pre- ventative capsulotomy during operation, while the control group （n=48） had no preventative capsuloto- nay. The degree of inflammation, the shape of anterior capsule, the diameter of the anterior capsule opening, the location of IOL, and the posterior capsule opacification were measured postoperatively until 6 months. Results There was no significant difference in the degree of inflammation between the two groups 1 day and l week after operation （P 〉0.05）. The diameter of the anterior capsule opening was significantly greater in the experimental group than in the control group （P 〈0.01）. No significant difference was found in the posterior capsule opacification （P 〉0.05）. And no IOL decen- tration or tilt was found in all patients. Conclusions Preventative capsulotomy in cataract surgery is effective in preventing anterior capsule contraction in patients with diabetes meUitus. And it does not increase the posterior capsule opacification, or other incidence of complications
