显微Parks切口斜视矫正术32例临床观察

摘　　要：目的探讨显微镜下Parks切口斜视矫正术治疗水平共同性斜视的优点。方法回顾分析2012年2月至2014年5月在苏州市吴中区越溪卫生院眼科通过显微镜下Parks切口斜视矫正术治疗的32例水平斜视患者，对部分患者行睫状前血管分离、保留，于手术后随诊6-12个月，对其术后眼位、结膜切口、手术并发症等进行观察和分析。结果术后眼部刺激症状轻，结膜切口愈合佳，睑裂暴露区结膜无手术瘢痕。术后眼位矫正治愈率93．75％（30／32），除1例大角度外斜的患者的术眼外转轻度受限，其余患者眼球运动正常。术中成功保留血管者16例。术中及术后均未出现严重并发症。结论显微镜下行Parks切口斜视矫正术治疗水平共同性斜视，术中术后并发症少，疗效确切，在基层医院值得推广。Objective To investigate the superiority of microsurgery for horizontal concomi- tant strabismus with Parks incision. Methods Retrospective analysis was done in 32 horizontal stra- bismus patients hospitalized in our department who underwent micro-operation and followed up for 8 to 12 months from February 2012 to May 2014. The anterior ciliary vessels were separated and re- served in some of them. Eye position, conjunctiva incision and surgical complications were observed after surgery. Results In postoperation eye irritation was weak, the conjunctival response was light and the incision healed well, and the conjunctive was no scar in the exposed area of the eyelid. The cure rate of eye position correction was 93.75% （30/32）. Except for 1 case of large angle exo- tropia patients with mild operation eye abducting limitation, the rest of them showed normal ocular movement. The ciliary vessels were too fine to give up the vessels separating in 7 cases and suc- cessfully preserved in 16 cases. There were no serious complications in operation and postoperation. Conclusions Micro-surgery for horizontal concomitant strabismus with Parks incision has the exact curative effect and has fewer complications. It is worthy to be popularized in grassroots level hospi- tals.
