F-PSA/T-PSA联合MRI在前列腺癌诊断中的应用价值

摘　　要：目的探讨MRI联合血清游离前列腺特异性抗原(F-PSA)以及总PSA(T-PSA)比值对于前列腺癌(PCa)的诊断价值。方法对病理确诊为前列腺良恶性疾病的103例患者,比较单独应用MRI以及MRI联合F-PSA/T-PSA诊断PCa的敏感性、特异性以及准确性。结果 103例患者中有39例患者被诊断为PCa,其中MRI诊断的敏感性、特异性以及准确性分别为89.74%、81.25%、85.87%,将MRI以及F-PSA/T-PSA比值联合诊断,发现选取0.15为截点时对PCa具有最大的诊断价值,其敏感性、特异性以及准确性分别为87.18%、89.06%、89.91%。结论 MRI联合F-PSA/T-PSA可以明显提高前列腺癌的诊断率,有助于早期发现前列腺癌并进行治疗。Objective To investigate the value of MRI combined with serum free prostate specific antigen （F-PSA） and total PSA （T-PSA） ratio in the diagnosis of prostate cancer. Methods 103 cases of benign and malignant prostate disease confirmed by pathological diagnosis were selected,sensitivity, specificity and accuracy of MRI and F-PSA/T-PSA MRI in the diagnosis of prostate cancer were compared. Results In 103 cases, 39 patients were diagnosed as prostate cancer, the sensitivity, specificity and accuracy of MRI diagnosis were 89.74%, 81.25 % , 85.87 % , MRI and F-PSA/T-PSA ratio combined diagnosis had the greatest diagnostic value on prostate cancer at cutoff point of 0. 15, the sensitivity, specificity and accuracy were 87. 18% , 89.06%,89.91%. Conclusion MRI combined with F-PSA/T-PSA can significantly improve the diagnostic rate of prostate cancer,and it is helpful in the early diagnosis and treatment of prostate cancer.
