摘 要:目的了解瞳距对斜视角度测量的影响并评估数字式弧形视野计应用于临床的可行性。方法 49例共同性斜视患者纳入本研究,其中共同性外斜视38例,共同性内斜视11例,分别使用传统弧形视野计、数字式弧形视野计和三棱镜+遮盖法测量患者远、近斜视角。对数字式弧形视野计和传统弧形视野计的测量结果进行比较,并分析两种弧形视野计测量结果和三棱镜+遮盖法的测量结果的相关性。结果测量近斜视角度时,数字式弧形视野计和传统弧形视野计测量结果没有差异(Z=-1.050,P>0.05)。测量远斜视角度时,对于瞳距未发育至成人水平的患者组(<13岁的女性及<15岁的男性),数字式弧形视野计和传统弧形视野计测量结果有差异(Z=-2.628,P<0.05),对于瞳距发育至成人水平的患者组(≥13岁的女性患者及≥15岁的男性患者),数字式弧形视野计和传统弧形视野计测量结果无统计学差异(Z=-0.038,P>0.05)。两组患者的数字式弧形视野计测量结果与三棱镜+遮盖法的相关系数均高于传统弧形视野计与三棱镜测量结果的相关系数(P<0.001)。结论瞳距会影响弧形视野计测量远斜视角度的结果,对于瞳距未发育至成人水平的患者数字式弧形视野计测量结果更准确。数字式弧形视野计是个可取的测量斜视角度的仪器。Objective To explore the impact of interpupillary distance on the measure of strabismus angle, and evaluate and the feasibility of the digital arc perimetry in clinical application. Method 49 Ophthalmic center's patients were involved, including 38 concomitant exotropia patients and 11 concomitant esotropia patients. The traditional curved perimeter, digital arc perimeter and prism plus cover test were used to measure patients' near squint angle and far squint angle. The validity between digital arc perimeter and traditional arc perimeter was compared and the difference of the two kinds of perimeter was analysis. Results There was no statistical difference between results of digital arc perimeter and the traditional arc perimeter in near squint angle measure(Z =-1.050,P〉0.05). For patients(less than 13-year-old female patient, and less than a 15-year-old male) whose interpupillary distance were under the adult levels, the difference between digital arc perimeter and traditional arc perimeter was statistically significant in far squint angle measure(Z =-2.628, P〈0.05). But for patients(more than 13-year-old female patients and more than a 15-year-old male) whose interpupillary distance had reached adult levels, the difference was no statistically significant(Z =-0.038, P〉 0.05). Digit arc perimeter's results were relevant to the prism plus cover method than traditional arc perimeter. Conclusion The result of far squint angle is affected by the pupil distance and digital arc perimeter is an effective instrument for measuring squint angle.
