亮丙瑞林治疗后血清黄体生成素水平评估中枢性性早熟患儿疗效的价值

摘　　要：目的探讨亮丙瑞林治疗后血清黄体生成素(LH)水平评估中枢性性早熟(CPP)患儿治疗效果的价值。方法女性CPP患儿16例,采用亮丙瑞林3.75mg治疗,每4周注射1次。分别于治疗前及治疗12、24周时检测注射后2h血清LH水平,结合生长发育、子宫B超及骨龄X射线片等指标综合评估疗效。结果与治疗前比较,治疗12周患儿乳房、卵巢、子宫等性腺发育呈减缓趋势,治疗24周发育进一步减缓(P<0.05),且骨龄/年龄比值降低,预测的成年期身高增加(P<0.05)。治疗12周和24周时的血清LH水平相仿(P>0.05),但均低于治疗前(P<0.05)。结论亮丙瑞林注射后2h,血清LH水平≤5.6IU/L,提示CPP患儿治疗效果较好。Objective To explore the value of luteinizing hormone（LH）level after leuprolide treatment in evaluating efficacy of central precocious puberty（CPP）.Methods Sixteen female children with CPP were treated with leuprolide 3.75 mg once every four weeks.Serum levels of LH at two hours after leuprolide injection were detected before and in the 12^th（T1）and 24^th（T2）week during treatment.The efficacy was evaluated by serum LH level combined with clinical indiators of growth and development characteristics,B ultrasonic of uterus and bone age.Results Compared with before,the clinical parameters of breast,ovarian and uterine were reduced at T1,which were significantly reduced at T2（P〈0.05）.Compared with before,the ratio of bone age to age was lower and the predicted adult height was more at T2（P〈0.05）.Serum levels of LH were significantly decreased at T1 and T2（P〈0.05）,which were not significantly different between at T1 and T2（P〈0.05）.Conclusion Serum level of LH at two hours after leuprolide injection≤5.6IU/L indicates that the treatment of leuprolide is effective in the girl with CPP.
