摘  要:目的研究自制细胞外基质胶泪道羊膜的生物活性因子表达含量、生物力学特征以及在修复兔泪道损伤中的应用效果。方法使用放射免疫分析方法对自制细胞外基质胶泪道羊膜中表皮生长因子(epidermal growth factor,EGF)、成纤维细胞生长因子(fibroblast growth factor,FGF)、肝细胞生长因子(hepatocyte growth factor,HGF)等10种生物活性因子的表达含量进行测定,并与新鲜泪道羊膜、干燥泪道羊膜进行对比。利用负荷传感器对比自制细胞外基质胶羊膜与新鲜泪道羊膜、干燥泪道羊膜的抗拉强度、断裂伸长率和弹性模力,且把保存12个月的自制细胞外基质胶泪道羊膜应用于修复兔泪道损伤模型,并于术后4周观察兔泪道黏膜修复及K16表达情况。结果自制细胞外基质胶泪道羊膜、干燥泪道羊膜中10种细胞因子含量均比新鲜泪道羊膜低(均为P<0.05),而ECM泪道羊膜中10种细胞因子含量均明显高于干燥泪道羊膜(均为P<0.05)。自制细胞外基质胶泪道羊膜的抗拉强度和断裂伸长率与新鲜泪道羊膜、干燥泪道羊膜相比明显增加(均为P<0.05),而自制细胞外基质胶泪道羊膜的弹性模量明显小于新鲜泪道羊膜及干燥泪道羊膜(均为P<0.05)。术后4周发现ECM-LDAM组泪道管口通畅,无明显的排斥反应和炎症反应,其下睑泪道黏膜K16表达阴性,而对照组泪道黏膜K16表达阳性。结论自制细胞外基质胶泪道羊膜含有丰富的生物活性因子和优越的生物力学特性,是一种更加安全有效的泪道修复及重建材料。Objective To study the biological factors and biomechanical characteristics of extracellular matrix lacrimal duct amniotic membrane,and observe its effects in the healing process of rabbit lacrimal duct injure. Methods The ten biological factors such as epidermal growth factor( EGF),fibroblast growth factor( FGF),hepatocyte growth factor( HGF) were analyzed by radioimmunoassay method,and the biomechanical characteristics including tensile strength,elongation at break and elastic modulus were compared by using electronic universal testing machine among extracellular matrix lacrimal duct amniotic membrane( ECM-LDAM),dry lacrimal duct amniotic membrane( D-LDAM) and fresh lacrimal duct amniotic membrane( F-LDAM).Besides,the ECM-LDAM which had been saved 12 months was applied to the model of rabbit lacrimal duct injure. Then the pathological changes and expression of K16 of lacrimal mucosa in rabbits were tested before surgery and 4 weeks after surgery. Results Compared with the D-LDAM and ECM-LDAM,the content of ten biological factors in F-LDAM were highest,and there was statistical significance( all P 〈0. 05). Then the content of ten biological factors in ECM-LDAM were higher than those in D-LDAM( all P 0. 05). The tensile strength and elongation at break of ECM-LDAM were higher than those of D-LDAM and F-LDAM( P 0. 05),the elastic modulus of ECM-LDAM was smaller than those of D-LDAM and F-LDAM( all P 〈0.05). Four weeks after lacrimal duct-repair surgery with ECM-LDAM in rabbits,the lacrimal duct pipe in ECM-LDAM group was clear without rejection reaction and inflammation,and the expression of K16 in lower lacrimal mucosa was negative,but it was positive in control group. Conclusion ECM-LDAM is rich in biological factors and has superiority on biomechanical characteristics,which is a safe material for repair and reconstruction of lacrimal duct.
