滋补汤联合卡培他滨维持治疗一线化疗后气血两虚证晚期结直肠癌的研究

摘　　要：目的观察滋补汤联合卡培他滨维持治疗一线化疗后气血两虚证晚期结直肠癌的疗效。方法将135例一线化疗后气血两虚证晚期结直肠癌患者随机分为3组,中西医结合组45例给予滋补汤联合卡培他滨治疗,中药组45例仅给予滋补汤治疗,对照组45例给予卡培他滨维持治疗。3组持续用药至肿瘤进展或出现无法耐受的不良反应为止。评估3组治疗前及治疗2个月后中医症状评分,治疗2个月后中医症状疗效及生活质量,统计3组无进展生存期(PFS)、疾病控制期(DDC)及毒副反应发生情况。结果中西医结合组和中药组治疗后中医症状评分明显低于治疗前及对照组(P均<0.05),生活质量提高比例明显高于对照组(P均<0.05);中西医结合组PFS时间明显长于中药组和对照组(P均<0.05),但3组DDC及不良反应发生情况比较差异均无统计学意义(P均>0.05)。结论滋补汤联合卡培他滨维持治疗气血两虚证晚期结直肠癌患者,能改善临床症状,提高生活质量,延长PFS。Objective It is to observe the curative effect of Nourishing soup combined with capecitabine in advanced colorectal cancer with syndrome of deficiency of both Qi and Blood after first-line chemotherapy. Methods 135 patients with advanced colorectal adenocarcinoma after first-line chemotherapy were randomly divided into TCM group,control group and combination group,45 cases in each group. The patients were treated with capecitabine in control group,and Nourishing soup in TCM group,and capecitabine combined with Nourishing soup in combination group. All the medication was continued to the tumor progression or adverse reactions cannot be tolerated. The TCM syndrome scores before treatment and after 2 months of treatment,and the curative effect of TCM symptoms and quality of life after 2 months of treatment were assessed in 3 groups;the progression free survival（ PFS）,disease control period（ DDC） and the incidence of adverse reaction in 3 groups were calculated. Results In the TCM group and the combination group,the TCM syndrome scores were significantly lower than before treatment and those in the control group（ all P 0. 05）,and the quality of life was significantly higher than that in the control group（ all P 0. 05）; the PFS of combination group（ 6. 5 months） was markedly longer than those of the TCM group（ 5months） and the control group（ 4. 5 months）（ all P 0. 05）,but there was no significant difference in DDC and the incidence of adverse reaction among the 3 groups（ all P 0. 05）. Conclusion Chinese medicine Nourishing soup combined with capecitabine can improve the TCM symptoms and the quality of life,and prolong the PFS time for advanced colorectal cancer with syndrome of deficiency of both Qi and Blood in maintenance treatment.
