摘  要:目的初步评价根尖定位仪在牙根纵折早期诊断中的应用价值。方法收集未做过牙髓治疗、临床诊断为牙根纵折或疑似纵折的患牙48颗,根尖定位仪测量根管工作长度。如工作长度较X光片上牙根长度短≥1/3者确诊为纵折。拔除牙根后,测量纵裂根管长度,并与定位仪测量结果进行比较。如定位仪测出的工作长度与X光片上牙根长度相差0.5~1 mm,则行常规小锥度根管预备和冷侧压充填、降合,观察1个月无症状后行冠修复。结果 48颗患牙中,41颗确诊为纵折,拔除后均可见裂隙贯穿根管,确诊率为85.4%。直视下测量的数值与根尖定位仪所测数值相比,差值均在1 mm以内。其余7颗疑似纵折牙中,4颗于修复后0.5~1年发现牙根纵折。结论根尖定位仪对牙根纵折的早期诊断具有一定的价值,但对于仅有裂线而无裂隙的患牙仍有漏诊的可能性。Objective To primarily evaluate the application value of root apex locator in the early diagnosis of vertical root fracture. Methods The root canal length of 48 non-endodontically treated teeth confirmed or suspected of vertical root fracture was measured by root apex locator. The roots with working length 1/3 of or much shorter than the primary length were diagnosed as vertical fracture. The actual length of vertical fracture root canal was measured after the uprooting of the teeth. The results were compared with that of the root apex locator. The roots with working lengths 0.5-1 mm shorter than the primary length were treated by routine root canal treatment and repaired with crowns after 1 month. Results Among the 48 teeth, 41 ones were diagnosed as vertical fracture, and fissures could be seen penetrating the root canals after the uprooting. The diagnostic rate was 85.4%. There was less than 1 mm difference in the values between the results of root apex locator and the actual length. Among the 7 teeth suspected of vertical root fracture, 4 ones were diagnosed as vertical root fracture 0.5 to 1 year after the reparation. Conclusion The root apex locator has a certain value in the early diagnosis of vertical root fracture. However, there is a possibility of missing diagnosis for the incomplete vertical root fracture.
