摘　　要：目的 :探讨CT对卵泡膜细胞瘤-纤维瘤组肿瘤和外突性子宫肌瘤的诊断及鉴别诊断价值。方法 :收集经病理证实的16例卵泡膜细胞瘤-纤维瘤组肿瘤和18例外突性子宫肌瘤的CT资料,观察比较2组肿瘤的CT征象。结果 :1卵泡膜细胞瘤-纤维瘤组肿瘤:实性成分CT值多低于子宫,多轻度强化或无强化,动脉期肿瘤内部可见纤细血管征,易合并腹水;肿块主要由卵巢动脉及子宫动脉卵巢支供血,部分可见卵巢血管蒂征。2外突性子宫肌瘤:实性成分CT值多接近或略高于子宫,多呈明显强化,可见假包膜,通常不合并腹水;肿块由子宫动脉供血,卵巢血管蒂征不常见。结论:卵泡膜细胞瘤-纤维瘤组肿瘤和外突性子宫肌瘤CT表现各有其特点,仔细观察2组肿瘤的CT表现,可以对两者进行诊断和鉴别诊断。Objective：To investigate the CT value in diagnosis and differential diagnosis of the group of ovarian thecoma-fibroma and outprojecting uterine leiomyoma. Methods：34 cases with the group of ovarian thecoma-fibroma and outprojecting uterine leiomyoma confirmed by surgical pathology. The CT features of two tumor groups were observed and compared carefully. Results： （1）The group of the ovarian thecoma-fibroma：the CT value of substantial part was lower than that of the uterus mostly,and had no or slight strengthening in enhanced scan;some cases displayed＂slim blood vessels＂ sign at artery stage; some cases displayed ascites;the feeding artery of tumor was ovarian artery or ovarian branch; a part of cases showed ＂ovarian vascular pedicle＂ sign. （2）The outprojecting uterine leiomyoma-the CT value of substantial part was close to or higher than that of the uterus mostly,and had obvious strengthening in enhanced scan; some cases displayed pseudo-capsule;ascites was not displayed; the feeding artery of tumor was uterine artery; ＂ovarian vascular pedicle＂ sign was seldom showed. Conclusion：The CT appearances of the ovarian thecoma-fibroma and outprojecting uterine leiomyoma have their own characteristics respectively. It is possible to diagnose and differentiate the two tumor groups correctly by observing the CT features carefully.
