针药结合治疗基底节区脑出血术后偏瘫临床观察

摘　　要：目的观察针药结合治疗对基底节区脑出血术后偏瘫患者肢体运动功能和日常生活能力的影响。方法采用回顾性研究方法,将100例基底节区脑出血术后偏瘫患者分成研究组和对照组,每组50例,对照组患者接受西医基础治疗,研究组患者在西医基础治疗基础上加用针刺治疗和加味补阳还五汤口服。比较两组患者的临床疗效,治疗前后分别采用简式Fugl-Meyer量表(Fugl-Meyer Assessment,FMA)和日常生活能力量表(activities of daily living,ADL)评价肢体运动功能和日常生活能力。结果两组临床疗效的分布比较,差异具有统计学意义(P<0.05),研究组临床疗效明显优于对照组;两组治疗后FMA和ADL评分均较治疗前显著增加(P<0.05),研究组FMA和ADL评分升高值显著大于对照组(P<0.05)。结论针刺疗法联合补阳还五汤内服可改善基底节区脑出血术后偏瘫患者的运动功能和生活自理能力。Objective To investigate the effect of acupuncture-medicine combination therapy on the motor function of extremities and activities of daily living in patients with hemiplegia after surgery for basal gan- glia hemorrhage. Methods The clinical data of 100 patients with hemiplegia after surgery for basal ganglia hemorrhage were analyzed retrospectively, and these patients were divided into study group and control group, with 50 patients in each group. The patients in the control group received basic Western medicine treatment, and those in the study group received acupuncture and oral administration of Jiawei Buyang Huanwu Decoction in addition to the basic Western medicine treatment. The clinical outcome was com- pared between the two groups, and the simplified Fugl-Meyer Assessment （FMA） and Activities of Daily Living （ADL） scales were used to evaluate the motor function of extremities and activities of daily living before and after treatment. Results The distribution of treatment outcomes showed a significant differ- ence between the two groups （P〈0.05）, and the study group had a significantly better clinical outcome than the control group. Both groups showed significant increases in FMA and ADL scores after treatment （P〈0.05）, but the study group had significantly greater increases in FMA and ADL scores compared with the control group （P〈0.05）. Conclusion Acupuncture combined with Jiawei Buyang Huanwu Decoction can improve the motor function and self-care ability of daily living in patients with hemiplegia after surgery for basal ganglia hemorrhage.
