针灸配合推拿治疗普通型偏头痛临床研究

摘　　要：目的比较针灸联合推拿疗法、单纯针灸、单纯推拿治疗普通型偏头痛的临床疗效。方法将90例普通型偏头痛患者随机分为针灸配合推拿治疗组(针推组)、针灸治疗组(针灸组)和推拿治疗组(推拿组),每组30例。7d为1个疗程,共治疗2个疗程。比较3组患者的临床疗效、中医证候积分及血浆降钙素基因相关肽(calcitonin gene-related peptide,CGRP)水平。结果 3组临床疗效的分布比较,差异具有统计学意义(P<0.05),针推组临床疗效明显优于针灸组和推拿组(P<0.05)。治疗后3组患者中医证候积分和血浆CGRP含量均较治疗前明显降低(P<0.05);3组治疗前后中医证候积分差值比较,差异均无统计学意义(P>0.05),针推组治疗后血浆CGRP含量降低值显著大于针灸组和推拿组(P<0.05)。结论针灸与推拿联用治疗偏头痛可提高疗效,其机制与降低血浆CGRP有关。Objective To compare the clinical effects of acupuncture, massage, and a combination of the two therapies in the treatment of ordinary migraine. Methods Ninety patients with ordinary migraine were randomly and equally divided into three groups： acupuncture group, massage group, and acupunc- ture-massage group. All patients received two 7-day courses of treatment. The treatment outcome, tradi- tional Chinese medicine （TCM） syndrome score, and plasma calcitonin gene-related peptide （CGRP） were compared between the three groups. Results There were significant differences in treatment outcome be- tween the three groups （P〈0.05）, and the acupuncture-massage group had a significantly better treat- ment outcome than the other two groups （P〈0.05）. The TCM score and plasma CGRP in all patients de- creased significantly after treatment （P〈0.05） ; the acupuncture-massage group had a significantly greater decrease in plasma CGRP than the other two groups （P〈0.05）, but there were no differences in the decrease in TCM score between the three groups （P〉0.05）. Conclusion A combination of acupuncture and massage has an improved clinical effect in the treatment of ordinary migraine, which may be associated with a greater decrease in plasma CGRP.
