摘  要:目的:分析比较常用的3种胸腹部肿瘤体位固定技术,探寻最为理想的一种,从而提高肿瘤的控制率。方法:胸腹部肿瘤患者共134例,其中胸部肿瘤患者78例、腹部肿瘤患者56例,分别采用热塑膜、负压垫、负压垫+热塑膜进行体位固定,其中热塑膜40例、负压垫44例、负压垫+热塑膜50例,于5、10、15、20 d治疗前进行电子射野影像验证,记录体位偏移数据进行分析。结果:在误差>5 mm范围内,热塑膜固定组X、Y、Z轴分别偏移10%、13%、8%;负压垫固定组X、Y、Z轴分别偏移9%、10%、10%;负压垫+热塑膜固定组X、Y、Z轴分别偏移6%、5%、5%。结论:为提高放射治疗精度,在胸腹部肿瘤放射治疗时应采用负压垫+热塑膜固定技术。Objective To compare the errors of three immobilization techniques in chest and abdomen tumor radiotherapy.Methods There were 78 chest tumor patients and 56 abdomen tumor patients divided into three groups using three immobilization techniques, of whom, 40 ones were included into the group immobilized with thermoplastic mask, 44 ones were in the group with vacuum bag, and 50 ones were in the group with vacuum bag and thermoplastic mask. The position offset data were recorded by EPID verification before 5, 10, 15 and 20 times of irradiation fraction. Results In case the error was more than 5 mm, the values of the offset of X, Yand Z axes of the thermoplastic mask group were 10%, 13% and 8% respectively, the ones of the vacuum bag group were 9%, 10% and 10% respectively, and the ones of the group with thermoplastic mask and vacuum bag were 6%, 5% and 5% respectively. Conclusion Vacuum bag and thermoplastic mask have to be involved in the immobilization during chest and abdomen tumor radiotherapy so that the radiotherapy accuracy can be enhanced.
