摘 要:目的探讨不同年龄组共同性外斜视行双眼外直肌后徙术后矫正视力,眼位及双眼单视功能的变化。方法将81例不同年龄组共同性外斜视患者分三组:3~6岁组31例,平均(4.1±0.3)岁;7~12岁组27例,平均(6.2±1.8)岁;≥13岁组23例,平均(25.4±2.2)岁,三组患者均行双眼外直肌后徙术后比较矫正视力变化、正位率、近距离立体视功能的提高率。结果 (1)矫正视力:仅7~12岁组中2例术后矫正视力提高,余均未变化;(2)正位率:术后6个月时,3~6岁组为97%,7~12岁组为93%,≥13岁组为91%,三组结果经卡方检验:X^2=5.815,P<0.05,差异有统计学意义;(3)近立体视提高率:3~6岁组为97%,7~12岁组为37%,≥13岁组为4%,三组结果经卡方检验:X^2=12.554,P<0.05,差异有统计学意义。结论手术时机对正位率及双眼单视功能重建影响大,应早期行手术治疗;双眼外直肌后徙术仍存在一定的欠矫率。Objective To explore the changes of corrected visual acuity, eye position and binocular vision of migration technique eyes in different age groups patients suffering the concomitant exotropia after bilateral external rectus recession. Methods The 81 concomitant exotropia patients were assigned into three groups according to ages: the average age of 31 patients in 3-6 year-old group was (4.1±0.3)years old; the average age of 27 patients in 7-12 year-old group was (6.2±1.8)years old; the average age of 23 patients in ≥13 years old group was (25.4±2.2)years old; orthophoria rate and stereopsis recovery rate were compared after bilateral external rectus recession. Results (1) The corrected visual acuity of two patients in the 7-12 age group improved, but the rest patients unchanged; (2) The anteroposterior rate of patients in 3-6 year-old group was 97%, 93% in 7-12 year-old group, 91% in ≥13 year-old group after 6 months, and the difference was statistically significant (χ2=5.815, P〈0.05, χ2 test); (3) Close stereopsis improved rate of patients in 3-6 year-old group was 97%, 37% in 3-6 year-old group, 4% in ≥13 year-old group, and the difference was statistically significant (χ2=12.554, P〈0.05, χ2 test). Conclusion It should be early surgical treatment, because timing of surgery is a great impact on eye position orthotropia and binocular vision recovery; there is a certain undercorrection rate after bilateral external rectus recession.
