单侧完全性唇腭裂患者颈椎姿势的研究

摘　　要：目的研究单侧完全性唇腭裂（UCLP）患者颈椎姿势的特点。方法选择30例婴幼儿期行唇腭裂修补术的恒牙列患者为试验组（男18例,女12例,年龄12~22岁,平均年龄16.7岁）,30例恒牙列期的非唇腭裂的个别正常颌患者为对照组（男16例,女14例,年龄12~24岁,平均年龄17.4岁）,测量自然头位下的头颅正位片和头颅侧位片。在头颅正位片上测量两组患者的第3至6节颈椎棘突点（C3,C4,C5,C6）偏离颅面中线的距离（C3-ML、C4-ML、C5-ML、C6-ML）;在头颅侧位片上测量颈椎平面CP与SN平面的交角（SN-CP）、颈椎切线CVT和齿突切线OPT与SN平面的交角（SN-CVT、SN-OPT）、颈椎切线CV与齿突切线OPT的交角（OPT-VCT）,对两组测量数据进行独立样本的t检验,研究UCLP颈椎姿势在冠状向和矢状向上的特点。结果试验组C3-ML、C4-ML、C5-ML、C6-ML较对照组的偏离程度大,P值分别为0.002、0.000、0.004、0.001,差异均有显著性（P〈0.01）;试验组SN-CP、SN-CVT、SN-OPT较对照组偏大,P值分别为0.025、0.019、0.006,差异均有显著性（P〈0.05）、CVT-OPT较对照组偏小,P值为0.002,差异亦有显著性（P〈0.01）。结论单侧完全性唇腭裂患者的颈椎较对照组更偏离颅面中线,颈椎更后倾,颈椎曲度小,头位更伸展。ObjectiveTo study the cervical vertebrae posture characteristics of the unilateral complete cleft lip and palate（UCLP）.Methods30 patients with permanent teeth were chosen as treatment group （18 male, 12 female; age range, 12-23 years; mean age 16.7 years）. All of them had their lip and palate cleft repaired in their infancy. 30 people with individual normal occlusion permanent teeth were chosen as control group （16 male, 14 female; age range, 12-24 years; mean age 17.4 years）. Lateral cephalometric radiographs and posterior-anterior （P-A） cephalograms with natural head position of both groups were measured. In P-A cephalograms, the distances from C3-C6 to facial midline were measured. In lateral cephalometric radiographs, the crossing angles of cervical vertebrae plane and SN plane （SN-CP）, cervical vertebrae tangent and SN plane （SN-CVT）, odontoid tangent plane and SN plane （SN-OPT）, and cervical vertebrae tangent and odontoid tangent plane （CVT-OPT） were measured. In order to study cervical posture of UCLP in the coronary and sagittal, Student＇st test were used to analyze the data of two groups. ResultsCompared with control group, the of distance from C3, C4, C5, C6 to the facial midline in UCLP were bigger,Pvalue was 0.002, 0.000, 0.004, 0.001, respectively, and there were significant differences among the groups （P〈0.01）. The angles of SN-CP, SN-CVT, SN-OPT in experimental group were bigger than those of control group,Pvalue was 0.025, 0.019, 0.006, respectively, and there were significant differences among the groups （P〈0.05）. But the angle of CVT-OPT was smaller than that of control group,Pvalue was 0.002 and there were significant differences among the groups （P〈0.01）.ConclusionCompared with control group, the cervical curvature vertebras of UCLP is more off-centered from facial mildline. The cervical curvature is smaller and the cervical vertebra posture is more backward among UCLP.
