摘 要:目的探讨抗抑郁治疗对晚期肺癌合并抑郁症患者化疗前后免疫功能的影响。方法将符合纳入标准的患者按随机数字表法分为治疗组和对照组,其中对照组19例,治疗组20例,治疗组予常规化疗方案化疗,并同时联合抗抑郁治疗,对照组单独给予常规化疗方案化疗,比较治疗前后2组T细胞亚群及NK细胞变化。结果治疗4周后治疗组与对照组相比CD3+、CD4+CD3+、CD8+CD3+、CD4+/CD8+和NK细胞均无明显差异(P>0.05);治疗8周后治疗组与对照组相比CD3+、CD4+CD3+、CD8+CD3+、CD4+/CD8+均有显著差异(P<0.05或P<0.01),NK细胞差异无统计学意义(P>0.05)。结论抗抑郁治疗对晚期肺癌合并抑郁症患者的淋巴细胞亚群有显著影响,推测抗抑郁治疗具有较好的效果,能显著提高患者的免疫功能,减轻化疗所致免疫损伤。Objective To investigate the effect of antidepressant therapy on immune function in advanced lung cancer patients with depression before and after chemotherapy.Methods Thirtynine advanced lung cancer patients with depression were randomly treated with conventional chemotherapy alone(control group,n=19)or in combination with antidepressant therapy(treatment group,n=20).T cell subsets and NK cells were observed before and after treatment.Results After treatment for 4 weeks,no significant differences were found in CD3+,CD4+CD3+,CD8+CD3+,CD4+/CD8+ and NK cells between the two groups(P〉0.05).After treatment for 8weeks,there were significant differences in CD3+,CD4+CD3+,CD8+CD3+ and CD4+/CD8+cells between the two groups(P〈0.01),but not in NK cells(P〉0.05).Conclusion Antidepressant therapy can significantly affect the lymphocyte subsets in advanced lung cancer patients with depression,suggesting that it can improve immune function and reduce chemotherapycaused immune injury.
