CHAF1A在肝细胞癌组织中的表达及其临床意义

摘　　要：目的研究染色质装配因子1(Chromatin assembly factor 1,CAF-1)的最大亚基p150(CHAF1A)在原发性肝细胞癌中的表达情况,并分析其表达与原发性肝细胞癌患者的性别、年龄、病理分化、门静脉癌栓、肿瘤大小、临床分期、血清HBsAg状况、肝硬化及血清AFP水平等临床病理特征的关系。方法筛选自2010年6月至2014年10月在南昌大学第一附属医院普外科行手术切除,并且全部经过病理诊断证实为原发性肝细胞癌的患者80例,取对应患者病灶边缘2cm处的正常肝组织80例及因外伤或肝内胆管结石行手术切除的正常肝组织患者20例分别作为癌旁和正常对照。应用免疫组织化学检测CHAF1A在肝癌组织、癌旁肝组织、正常肝组织中的表达,采用χ2检验分析CHAF1A在这三组中的表达差异,并分析CHAF1A的表达与肝癌临床病理特征的关系。结果CHAF1A在正常肝组织和癌旁肝组织的高表达率显著高于肝癌组织(P<0.05)。CHAF1A在不同病理分化肝癌组织中的高表达率差异有统计学意义(P=0.027);在有无伴门静脉癌栓肝脏组织中,高表达率差异有统计学意义(P<0.05)。CHAF1A在肝癌组织中的表达与性别、年龄、临床分期、肿瘤大小、血清HBsAg、肝硬化及血清AFP水平无相关性(P>0.05)。结论 CHAF1A的表达可能影响肝癌组织分化和侵袭性。但与肝癌患者的性别、年龄、临床分期、肿瘤的大小、HBsAg状况、肝硬化及血清AFP水平无相关性。Objective Research the expression of largest subunit p150 of Chromatin assembly factor 1（CAF-1）（CHAF1A）in hepatocellular carcinoma（HCC）,and analyze the ralativity between its expression and clinical pathological characteristics of HCC patients.such as age,gender,pathological differentiation,portal venous tumor emboli,tumor size,clinical stage,HBsAg situation,cirrhosis of the liver and serum AFP.Methods Screening 80 patients,all of them from June2010 to October 2014 have accepted surgical resection in the first affiliated hospital of Nanchang university general surgery,and are confirmed by pathological diagnosis to be HCC.Tumor tissues and normal liver tissues 2cm far near the cancer and normal liver tissues of 20 patients due to trauma or intrahepatic biliary calculi surgical resection of tissues are collected.Expression of CHAF1 Ain liver cancer are detected by immunohistochemistry.The differences of CHAF1 Aexpression in the three groups and the relationship between expression of CHAF1 A with clinical pathological characteristics of patients.Results The high expression rate of CHAF1 Ain paraneoplastic and normal liver tissue was significantly higher than that in liver cancer tissue（P〈0.05）.CHAF1 Aexpression in poorly differentiation liver cancer tissue or liver cancer tissue associated with portal venous tumor emboli is lower than high differentiation liver cancer tissue（P=0.027）or no portal venous tumor emboli（P〈0.05）.The expression of CHAF1 Ain liver cancer tissue is not related with gender,age,clinical stage,tumor size,HBsAg,cirrhosis of the liver and serum AFP Of patients（P〉0.05）.Conclusion CHAF1 Aexpression maybe affect differentiation and invasiveness of liver cancer.The expression of CHAF1 Ain liver cancer tissue is not related with gender,age,clinical stage,tumor size,HBsAg,cirrhosis of the liver and serum AFP Of patients.
