新辅助化疗对局部进展期乳腺癌患者近期疗效及T淋巴细胞免疫功能的影响

摘　　要：目的对比多西他赛联合表柔比星(TE)和多西他赛联合表柔比星及环磷酰胺(TEC)方案在乳腺癌新辅助化疗(NAC)中的近期疗效,分析化疗前后T淋巴细胞亚群免疫功能变化。方法 51例Ⅱ-Ⅲ期乳腺癌患者随机分为TE方案组和TEC方案组,其中TE组25例,TEC组26例;分别接受TE和TEC方案化疗;化疗结束后评价疗效,并采用流式细胞术检测51例患者新辅助化疗前后的静脉血T淋巴细胞亚群,分析其与临床疗效的相关性。结果 TE组与TEC组的有效率分别为80.0%和84.6%,差异无统计学意义(P>0.05)。TE与TEC有效组CD3+、CD4+、CD4+/CD8+细胞较治疗前均升高(P<0.05),CD8+降低(P<0.05);无效组CD3+、CD4+/CD8+较治疗前降低(P<0.05),而CD8+升高(P<0.05)。结论乳腺癌患者免疫功能降低,新辅助化疗可提高免疫功能;监测免疫功能,有助于化疗药物疗效的判断。Objective To compare and analyze the effect of the two noadjuvant chemotherapy（ NAC） regimen（ TE and TEC）. And To explore the functional changes of T lymphocyte subsets before and after neoadjuvant chemotherapy in patients with locally advanced breast cancer. Methods The data of 51 female patients with stage II and III breast cancer was retrospectively analyzed. All patients were received NAC with TE or TEC regime before operation. T lymphocyte was measured by flowcytometry in patients with locally advanced breast cancer before and after neoadjuvant chemotherapy. Results Between the two gropes,the rate of RR were 80. 0% and 84. 6%,there were no significant differences. After the NAC,CD3＋,CD4＋,CD4＋/CD8＋were higher in response group than those in pretreatment samples（ P〈0. 05）,while CD8＋cells were lower than those in pretreatment samples（ P〈0. 05）. CD3＋,CD4＋,CD4＋/ CD8＋were lower in non- response group than those in pretreatment samples（ P〈0. 05）,while CD8＋cells were higher than those in pretreatment samples（ P〈0. 05）. Conclusion The effects of the two NAC regiments are similar.The immune function was low in the patients with locally advanced breast cancer. The immune function of the patients could be increased by effective NAC. It may be helpful in the clinical monitoring and treatment by detecting the immune function.
