摘 要:目的检测Toll样受体9(TLR9)和去整合素-金属蛋白酶(ADAM)17蛋白在人食管鳞癌组织中的表达,探讨两者在食管鳞癌发生、发展、浸润、转移中的作用以及两者间的相关关系,并进一步对其与食管癌的临床及病理特征间的关系进行分析。方法通过免疫组织化学Envision^(TM) plus法检测2000年3月至2008年7月复旦大学附属上海市第五人民医院胸外科经手术治疗的81例食管鳞状细胞癌患者的存档蜡块肿瘤组织和30例癌旁正常组织中TLR9、ADAM17蛋白的表达情况。结果在食管鳞癌组织中TLR9、ADAM17的表达率分别为69.1%(56/81)和72.8%(59/81),而在癌旁组织中TLR9、ADAM17的表达率分别为30.0%(9/30)和26.7%(8/30),差异有统计学意义(P<0.01);TLR9、ADAM17在食管癌组织Ⅲ+Ⅳ期中的表达率显著高于Ⅰ+Ⅱ期[80.4%(37/46)比54.3%(19/35),84.8%(39/46)比57.1%(20/35)],在T_3+T_4中的表达率显著高于T_1+T_2[85.7%(36/42)比51.3%(20/39),83.3%(35/42)比61.5%(24/35)],在有淋巴结转移中的表达率显著高于无淋巴结转移组织[79.2%(38/48)比54.5%(18/33),83.3%(40/48)比57.6%(19/33)],差异均有统计学意义(P<0.05)。且两者的表达之间呈正相关(r=0.372,P<0.05)。结论TLR9和ADAM17在食管鳞癌组织中高表达;TLR9和ADAM17在食管鳞癌组织中的表达呈正相关,联合检测TLR9和ADAM17在食管鳞癌组织中的表达可作为临床判断食管癌的分期、分级参考依据。Objective To detect the expression of Toll-like receptor 9( TLR9) and adisintegrin and metalloproteinase 17( ADAM17) protein in human esophageal squamous carcinoma tissues to explore the role of TLR9 and ADAM17 in the occurrence,development,invasion and metastasis of esophageal squamous carcinoma and the relationship between the two indicators,and further analyze the relationship with clinical and pathological features of esophageal cancer. Methods Expressions of TLR9 and ADAM17 protein were examined by immunohistochemistry EnvisionTM plus from 81 cases of esophageal squamous carcinoma and 30 cases of adjacent non tumor tissue which were from the Department of Chest Surgery of the Fifth People’ s Hospital of Shanghai,Fudan University from Mar. 2000 to Jul. 2008. Results The positive rate of TLR9 and ADAM17 protein in adjacent non tumor tissue was 30%( 9 /30) and 26. 7%( 8 /30) respectively,and that in esophageal squamous carcinoma was 69. 1%( 56 /81) and 72. 8%( 59 /81) respectively. There were significant differences between adjacent non tumor and esophageal carcinoma tissues( P 〈 0. 01). TLR9,ADAM17 expression rates in esophageal tissue stageⅢ + Ⅳ were significantly higher than in esophageal tissue stageⅠ + Ⅱ[80. 4%( 37 /46) vs 54. 3%( 19 /35),84. 8%( 39 /46) vs 57. 1%( 20 /35) ]; expression rates in the tissue with lymph node metastasis were significantly higher than in the tissue without lymph node metastasis [79. 2%( 38 /48) vs 54. 5%( 18 /33),83. 3%( 40 /48) vs 57. 6%( 19 /33) ],the differences were statistically significant( P 〈 0. 05). TLR9 expression was positively correlated with ADAM17( r = 0. 372,P 〈0. 05). Conclusion TLR9 and ADAM17 are highly expressed in esophageal squamous carcinoma tissues;the expression of TLR9 and ADAM17 in esophageal squamous cell carcinoma tissue is positively correlated.Joint detection of the expression of TLR9 and ADAM17 in esophageal squamous cell carcinoma can be used as reference for
