摘 要:目的探讨早孕期(孕11-13+6周)系统超声切面检查在胎儿结构畸形筛查方面的应用价值。方法对2014年1月—12月在东莞市妇幼保健院行超声检查的10 669名孕妇共10 883例孕11-13+6周胎儿进行系统超声切面检查,并对比中晚孕期超声及出生后体检结果。结果孕期超声共检出115例胎儿共154处结构畸形(包括1例胎儿单处结构畸形及1例胎儿多处结构畸形,多发畸形综合征计为1处结构畸形),产后体检发现孕期超声漏诊3例结构畸形。其中早孕期超声检出62例胎儿共85处结构畸形,早孕期超声诊断胎儿结构畸形敏感度为54.1%(85/157)。结论早孕期系统超声切面检查可以发现相当一部分胎儿结构畸形,尤其针对重大致死性结构畸形。早孕期系统超声检查对于提早部分胎儿畸形的诊断时间是行之有效的。Objective To discuss the application value of system ultrasonic inspection in the early pregnancy(11 ~13 +6weeks of pregnancy) in the screening for fetal structural abnormalities. Methods 10 883 cases of fetuses of 11~13+6 weeks of pregnancy from 10 669 pregnant women receiving ultrasonic examination in Dongguan maternal and child care service centre from January 2014 to December 2014 were given system ultrasonic inspection, pregnancy ultrasound in the middle and late stage and physical examination results after birth were compared. Results Pregnancy ultrasound detected that there were 154 structural abnormalities in total(1 case with single structural abnormality,1 case with multiple structural abnormalities and multiple anomaly syndrome was for structural abnormality), postpartum physical examination found that there were 3 cases of missed diagnosis by pregnancy ultrasound with structural abnormalities, early pregnancy ultrasound detected that there were 85 structural abnormalities from 62 cases of fetuses, the sensitivity of fetal structural abnormalities was54.1%(85/157)by early pregnancy ultrasound diagnosis. Conclusion System ultrasonic inspection in the early pregnancy can find quite a number of fetuses structural abnormalities, especially fatal structural abnormalities, it is feasible to make the diagnostic time of partial fetal abnormalities early by system ultrasonic inspection in the early pregnancy.
