微小切口进路坚固内固定治疗口腔颌面部骨折临床分析

摘　　要：目的分析运用微小切口进路坚固内固定方法治疗口腔颌面部骨折的临床效果。方法随机选取2000年5月—2013年5月前来该院就诊的60例口腔颌面部骨折患者为研究对象,采取随机抽取的方式将患者分为观察组和对照组,每组30例患者,对观察组的患者进行微小切口进路坚固内固定方式进行口腔颌面部骨折治疗;对对照组的患者进行传统颌间结扎方式进行口腔颌面部骨折治疗,比较两组患者的治疗效果。结果手术后,观察组患者愈合程度、咬合关系,张口度,外形恢复等皆优于对照组患者术后效果,差异有统计学意义(P<0.05);观察组患者的并发症总发生率5.3%明显低于对照组的18.7%,组间比较差异有统计学意义(P<0.05)。结论对口腔颌面部骨折行微小切口进路坚固内固定治疗在外形恢复、口腔颌面咬合和张口关系、骨折恢复和钛板移位上的效果更加显著,值得在临床治疗上积极推广。Objective To analyze the effect of rigid internal fixation with small incision approach in the treatment of oral and maxillofacial fracture. Methods 60 cases with oral and maxillofacial fractures visited our hospital from May 2000 to May 2013 were selected as the subjects and randomized into the observation group and the control group with 30 cases in each. Patients in the observation group were treated by rigid internal fixation with small incision approach, and those in the control group were treated by intermaxillary wiring with the traditional method. And the treatment effect of the two groups was compared. Results After surgery, the degree of healing, occlusion, mouth opening, recovery of shape and so on was significantly better in the observation group than that in the control group, respectively, with a statistically significant difference（P〈0.05）. The overall incidence of postoperative complications was much lower in the observation group than that in the control group（5.3% vs 18.7%） with a statistically significant difference（P〈0.05）. Conclusion Using rigid internal fixation with small incision approach in the treatment of oral and maxillofacial fracture achieves more significant effect in shape recovery, oral and maxillofacial occlusion and mouth opening relationship, fracture recovery and the titanium shift, which is worth actively promoting in the clinical treatment.
