摘  要:目的系统分析α1-抗胰凝乳蛋白酶(ACT)基因多态性与脑出血之间的关系。方法系统检索PubMed(1990.1-2014.58)、Embase(1967.1-2009.5)数据库及维普(1989-2009)、CNKI(1979-2009)中文期刊数据库,并通过阅读相关综述及文章的参考文献进一步获取信息,对目前已发表的评估ACT基因多态性与脑出血关系的候选基因关联研究进行荟萃分析。应用随机效应模型计算比值比(OR)及其95%可信区间(CI),用以描述ACT基因多态性与脑出血的关系。结果共有7项探讨ACT基因同一多态性位点与脑出血关系的研究(合计脑出血病例815例,对照1121例)纳入荟萃分析。我们按照种族分为高加索人和亚洲人两个种群,结果显示在高加索人中的比值比OR=1.43,95%CI1.00~2.13;亚洲人中的比值比OR=1.14,95%CI0.89~1.48。异质性在高加索人(I^2=48.3%,P=0.122)中比亚洲人(I^2=84.5%,P=0.002)中要小。结论高加索人ACT基因多态性与脑出血的易感性增加可能有关。Objective In recent years, several data showed that the TT genotype of the α1-antichy- motrypsin(ACT) gene polymorphism were associated intracerebral hemorrhage, but they have some conflicts, so we performed a meta-analysis to confirm their relationship. Methods Studies were identified through PubMed, Embase, CNKI, reviews, and references lists of relevant papers. Pooled odds ratios (OR) with corresponding 95% confidence intervals(CI) were calculated to express the associations by using fixed-effect models. Heterogeneity between studies was tested using the Q statistic. Results 7 studies (815 intracerebral hemorrhage cases, 1121 controis) that investigated the were included and evaluated. We divided them into two group, for Caucasian, ACT gene polymorphism may associated with intracerebral hemorrhage (OR = 1.43, 95%CI 1.00-2.13 ), for Asian OR= 1.14, 95%CI 0.89-1.48. The heterogeneity in Caucasian (I^2=48.3%, P=0.122) was lower than in Asian (I^2= 84.5%, P=0.002). Conclusion This meta-analysis suggested for Caucasian ACT gen polymorphism may associated with intraeerebral hemorrhage.
