术前血清C-反应蛋白水平在卵巢癌患者预后判断中的价值

摘　　要：[目的]探讨术前卵巢癌血清中C-反应蛋白(C-reactive protein,CRP)与临床病理参数及预后的关系。[方法]回顾性分析2008年1月至2014年4月手术的187例初治卵巢癌患者的病例资料,分析术前CRP值与年龄、国际妇产科联盟(International Federation of Gynecologists and Obstetricians,FIGO)分期、组织分级、腹水量及术后残余病灶大小的关系。[结果]82例(44.09%)卵巢癌患者术前血清CRP值为阳性,平均值为26.41 mg/L。CRP值与FIGO分期(P<0.05),残余肿瘤大小(P<0.01)和腹水量(P<0.01)显著性相关。术前血清CRP阳性率与腹水量相关(P<0.01),与年龄、组织分级和FIGO分期无关。CRP阳性的卵巢癌患者5年生存率(62.3%)低于CRP阴性的患者(79.6%)(P<0.01)。[结论]检测术前血清CRP值对卵巢癌病情严重程度及预后有重要临床意义。[Purpose] To investigate the correlations between preoperative C-reactive protein（CRP）serum concentration and the clinical pathological parameters as well as prognostic value in patients with ovarian cancer. [Methods] A total of 187 ovarian cancer patients were selected from January2008 to April 2014,and clinicopathological and C-reactive protein data were analyzed. Correlation between CRP expression and age,International Federation of Gynecologists and Obstetricians（FIGO） staging,histological grade,ascites and size of postoperative residual lesions were evaluated.[Results] Eighty-two（44.09%） cases of ovarian cancer were CRP-positive with an average serum CRP concentration of 26.41mg/L. Non-parametric and multivariate analyses showed significant correlation between CRP concentrations and FIGO staging（P〈0.05）,residual tumor（P〈0.01） and ascites（P〈0.01）. The positive rate of preoperative serum CRP was positively associated with ascites（P〈0.01）,but not associated with age,tissue grade and FIGO staging. The five year survival rate of CRP positive patients with ovarian cancer（62.3%） was significant lower than that of the CRP nega-tive patients（79.6%）（P〈0.01）. [Conclusion] Detecting the serum CRP concentration may be useful in clinical value for evaluating the disease severity and prognosis in patients with ovarian cancer.
