高血压脑出血患者术后肺部感染病原菌分布与对肺功能的影响分析

摘　　要：目的分析高血压脑出血患者术后肺部感染病原菌分布,并探讨感染后患者肺功能的变化,为临床治疗提供参考依据。方法选择医院2012年1月-2014年12月诊治的60例高血压脑出血术后肺部感染患者为感染组,并分析患者感染后肺功能的变化,同期选取医院健康体检者60名为对照组,高血压脑出血未感染患者60例为未感染组,对比3组受试者临床资料,数据采用SPSS 20.0软件进行统计分析。结果共分离病原菌60株,以革兰阳性菌为主,共34株占56.7%;感染病原菌后患者用力呼气容积(FEV1)、用力肺活量(FVC)及FEV1/FVC及FEV1%明显降低,且动脉二氧化碳分压水平明显升高,而动脉氧分压水平明显降低,且与对照组和未感染组患者比较,差异有统计学意义(P<0.05)。结论高血压脑出血术后患者肺部易受到病原菌感染,且感染后患者肺功能受到明显影响。OBJECTIVE To explore the pathogen distribution and effects on pulmonary function in hypertensive intracerebral hemorrhage related pulmonary infection patients,and to provide reference for clinical treatment.METHODS A total of 60 cases of pulmonary infected patients after hypertensive intracerebral hemorrhage in our hospital from Jan.2012 to Dec.2014 were enrolled in this study.The pathogen distribution and pulmonary function were detected.At the same time,60 cases of healthy subjects and 60 cases of non-infected patients were taken as control.Clinical data of the three groups were compared and the results were statistically analyzed with software SPSS 20.0.RESULTS A total of 60 pathogens were isolated from the 60 infected patients after hypertensive intracerebral hemorrhage that contained 34（56.7%）gram-positive bacteria.The results also revealed that the pulmonary function related parameters FEV1,FVC,FEV1/FVC and FEV1% as well as PaO_2 were decreased greatly,while PaCO2 content increased greatly after infection when compared with the control.The difference was significant（P〈0.05）.CONCLUSIONPulmonary infected patients after hypertensive intracerebral hemorrhage are vulnerable to pathogen infection and the pulmonary function is interfered greatly after infection.
