老年急性脑血管疾病患者肺部感染的病原菌分布与药敏分析

摘　　要：目的探讨老年急性脑血管疾病患者肺部感染的病原菌分布及药敏性,为临床治疗提供参考依据。方法选择2010年1月-2014年12月医院收治的72例老年急性脑血管疾病合并肺部感染患者为研究对象,通过采集患者的深部咳痰进行培养病原菌,观察患者的病原菌分布及药敏情况,数据采用SPSS 18.0软件进行统计分析。结果患者的痰液标本培养出163株病原菌,以革兰阴性菌为主,共107株占65.6%,其中以大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌为主;大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌对青霉素耐药率最低,均为0,对亚胺培南的耐药率最高,均>90.0%。结论老年急性脑血管疾病合并肺部感染的主要病原菌为革兰阴性菌,在药物选择上应选择耐药率较低的药物如头孢唑林、青霉素、万古霉素进行治疗,从而控治感染,减少新的耐药菌出现。OBJECTIVE To investigate distribution and drug sensitivity pathogens causing pulmonary infections in elderly patients with acute cerebrovascular disease.METHODS Totally 72 cases of elderly patients with acute cerebrovascular disease combined with pulmonary infections treated from Jan.2010 to Dec.2014 in our hospital were selected for the study.Deep sputum of patients were collected and used for culturing pathogens.Distribution and drug sensitivity of pathogens were observed.Data were analyzed with SPSS 18.0.RESULTS The patients sputum specimens cultured 163 pathogens,including gram-negative bacteria 107strains（65.6%）as the main pathogen.The main pathogens were Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa.Drug resistant rates of Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa towards penicillin were 0,while their resistant rates towards imipenem were over 90.0%.CONCLUSIONThe main pathogens causing pulmonary infection in elderly patients with acute cerebrovascular disease were gram-negative bacteria.It is beneficial to choose drugs with lower drug resistance rates,such as cefazolin,penicillin,vancomycin for treatment,thereby control infections,and reduce the emergence of new drug-resistant pathogens.
