摘  要:目的研究轻度认知功能障碍(MCI)各亚型血清神经元特异性烯醇化酶(NSE)水平差异,分析其在MCI诊断中的价值。方法选择MCI患者127例(MCI组)、阿尔茨海默病(AD)患者47例(AD组)及43例正常老年人(正常对照组),MCI组分为遗忘型MCI(a-MCI)42例,血管型MCI(V-MCI)54例,帕金森病型MCI(PD-MCI)31例。用ELISA法检测血清NSE水平,与2年随访NSE比较,记录MCI向AD的转化率。结果 MCI组、AD组及正常对照组NSE分别为(16.5±6.3)μg/L、(29.5±8.2)μg/L、(6.4±2.4)μg/L,MCI和AD组与正常对照组NSE比较均差异显著(P<0.01),AD组明显高于MCI组(P<0.01)。入院时MCI组中a-MCI[(11.3±3.7)μg/L vs(15.7±5.8)μg/L,P<0.05]、V-MCI[(25.3±5.1)μg/L vs(32.5±8.9)μg/L,P<0.01]、PD-MCI[(12.9±4.9)μg/L vs(20.7±6.4)μg/L,P<0.01]NSE均较2年随访降低,差异有统计学意义。相同时期,V-MCI和PD-MCI与a-MCI的NSE比较,差异有统计学意义(P<0.01)。2年随访a-MCI、V-MCI、PD-MCI转化为AD的概率分别为66.7%、36.8%、25.0%,a-MCI与V-MCI和PD-MCI转化率比较差异有统计学意义(P<0.01)。结论 NSE在MCI的发生发展过程中逐渐升高,结合临床神经系统检查,血清NSE可能为MCI早期诊断提供重要参考。objective To analyze the value of serum NSE levels in diagnosis of mild cognitive impairment(MCI)by measuring them in patients with different subtypes of MCI.Methods One hundred and twenty-seven MCI patients served as MCI group,47 AD patients served as AD group and 43 health elderly persons served as NC group in this study.Their serum NSE levels were measured.The patients were followed up for 2years during which their serum NSE levels were comaparatively analyzed.The 127 patients in MCI group were further divided into amnestic MCI(a-MCI)group(n=42),vascular MCI(V-MCI)group(n=54)and PD-MCI group(n=31).The transformation rate of MCI to AD was recorded.Results The NSE level was significantly higher in MCI group and AD group than in NC group and in AD group than in MCI group(P〈0.01).The serum NSE level was 11.3±3.7μg/L,25.3±5.1μg/L,12.9±4.9μg/L and 15.7±5.8μg/L,32.5±8.9μg/L,20.7±6.4μg/L respectively in a-MCI group,V-MCI group and PD-MCI group on admission and at the end of 2-year follow-up(P〈0.05,P〈0.01).The transformation rate of MCI to AD was 66.7%,36.8%and 25.0%respectively in a-MCI group,V-MCI group and PD-MCI group(P〈0.01).Conclusion The serum NSE level increases with the development and progression of MCI and can thus be used as an important parameter in early diagnosis of MCI.
