直肠癌Miles手术行经腹直肌乙状结肠造口对减少术后结肠造口旁疝发生率的作用

摘　　要：目的探讨直肠癌Miles手术行经腹直肌乙状结肠造口对减少术后结肠造口旁疝的效果。方法选取2012年6月至2014年12月在茂名市高州中医院行直肠癌Miles手术的98例患者为研究对象,用随机数字表法将98例患者分为观察组与对照组,每组49例。对照组接受传统腹膜内结肠造口术治疗,观察组给予经腹直肌乙状结肠腹膜外造口术进行治疗,观察二组患者周围炎、结肠旁疝、造口回缩等指标变化情况。结果观察组造口结肠旁疝(4.1%)、周围炎(2.0%)、造口回缩(2.0%)发病率明显低于对照组(22.4%、14.3%、18.4%),二组数据比较差异均具有统计学意义(P<0.05)。结论经腹直肌乙状结肠造口术具有并发症少、功能恢复快等优点,能有效提升患者的生活质量,值得在临床上推广使用。Objective To explore the Miles rectal cancer surgery abdominal rectus sigmoid colostomy to reduce the effect of postoperative beside hernia underwent colostomy. Methods From June2012 to December 2014,98 cases of rectal cancer who underwent Miles operation in Gaozhou Hospital of Traditional Chinese Medicine were selected in this study. The patients were randomly divided into observation group and control group： 49 cases in each group. Patients in control group received traditional intraperitoneal colostomy,and the transrectus extraperitoneal sigmoid colostomy was given in observation group. Control group accepted traditional peritoneal colostomy in treatment,the observation group was given by the rectus sigmoid colon extraperitoneal colostomy surgery treatment. Peripheral inflammation,next to the colon hernia, colostomy retraction were observed. Results Colostomy colon hernia, peripheral inflammation,colostomy retraction of observation group were 4. 1%,2. 0%,2. 0%,were higher than those in control group（ 22. 4%,14. 3%,18. 4%）,the difference was statistically significant（ P〈0. 05）.Conclusion Transrectus sigmoid colostomy has advantages of less complications, faster recovery of defecation function,can effectively improve the quality of life of patients,and it is worth popularizing in clinical use.
