腹腔镜子宫肌瘤切除术对患者卵巢功能及血清学指标的影响

摘　　要：目的:探讨腹腔镜子宫肌瘤切除术对患者卵巢功能及血清学指标的影响。方法:回顾分析2013年6月至2015年6月收治的78例子宫肌瘤切除患者的临床资料,根据患者要求分为腹腔镜组与开腹组,每组39例。对比分析两组患者手术前后卵巢功能及手术相关创伤指标。结果:术后腹腔镜组患者血清雌二醇表达水平高于开腹组,而卵泡刺激素、黄体生成激素表达水平较开腹组低(P<0.05);腹腔镜组患者术后血清炎性因子白细胞介素-6、肿瘤坏死因子-α均较开腹组低,而白细胞介素-10的表达水平却高于开腹组(P<0.05);腹腔镜组血清中CD3^+、CD4^+、CD8^+细胞表达水平及CD4^+/CD8^+均较开腹组高(P<0.05)。结论:腹腔镜子宫肌瘤切除术较开腹子宫肌瘤切除术机体的应激小、创伤小,利于患者术后康复,值得临床推广应用。Objective： To explore the influence of laparoscopic myomectomy on ovarian function and serum related indexes.Methods： The clinical data of 78 patients with uterine leiomyoma were retrospectively analyzed from Jun. 2013 to Jun. 2015. The 78 patients were divided into laparoscopic group and laparotomy group according to patients＇ demand. The surgical trauma index and ovarian function before and after operations were compared between the two groups. Results： The serum level of estradiol was higher in the laparoscopic group than that in the open group,while the expression of luteinizing hormone and follicle-stimulating hormone was lower than that in the open group（ P 0. 05）. About the levels of serum interleukin-6 and tumor necrosis factor-α,the laparoscopic group was lower than the open group; however,the levels of serum interleukin-10 was higher than the open group with significant differences（ P 0. 05）. The expression levels of CD3＋,CD4＋,CD8＋cells and CD4＋/ CD8＋in laparoscopic group were higher than those in the open group（ P 0. 05）. Conclusions： Compared with open hysteromyomectomy,laparoscopic hysteromyomectomy has less body stress and smaller trauma,which is beneficial to postoperative recovery,it is worth popularization in clinical practice.
