不同时间导泻对急性重症胰腺炎患者疗效影响

摘　　要：目的探讨急性重症胰腺炎患者不同时间导泻的临床效果及对预后的影响。方法从临床收治的急性重症胰腺炎患者中选取72例为研究对象,经随机数字表法,分为观察组与对照组各36例,观察组在发病48h内给予早期导泻治疗,对照组在发病48h后给予导泻治疗,观察两组患者血清淀粉酶复常、发热消退、腹痛及腹胀缓解、压痛消失时间,血、尿淀粉酶酶恢复时间,住院时间及费用,并发症发生情况及死亡情况。结果观察组患者血清淀粉酶复常、发热消退、腹痛及腹胀缓解、压痛消失时间、血尿淀粉恢复时间与对照组比较,均明显较短(P<0.05);观察组患者住院时间明显短于对照组(P<0.05),住院费用明显低于对照组(P<0.05);两组患者死亡率无明显差异(P>0.05),但观察组并发症发生率为12.82%,明显低于对照组的25.64%(P<0.05)。结论对于急性重症胰腺炎患者,在早期进行导泻治疗,可有效促进症状改善,缩短恢复时间,减少住院费用,降低并发症发生率和改善预后,可在临床推广应用。Objective To investigate the clinical effect of catharsis at different time on patients with severe acute pancreatitis.Methods 72 severe acute pancreatitis patients were divided into the observation group and the control group.The observation group was treated with catharsis within 48 hours after onset.The control group was treated with catharsis 48 hours after onset.Serum amylase,fever,abdominal pain,abdominal distension,recovery time of blood,urine amylase,hospitalization time,cost,complications and mortality were observed.Results The serum amylase,fever,abdominal pain,abdominal pain and abdominal distension,pain relief,time of pain relief,and the recovery time of blood and urine of observation group were significantly shorter than those of control group（P〈0.05）.The mortality was not significantly different between the two groups（P〈0.05）,but the incidence of complications of observation group（25.64%）was significantly lower than that of control group（P〈0.05）.Conclusion For patients with severe acute pancreatitis,early cathartic therapy can effectively promote the improvement of symptoms,shorten the recovery time,reduce the cost of hospitalization,decrease the incidence of complications and improve the prognosis.
